Nitric oxide supplements—which ones work, and which are empty marketing?

Sources: 13 · Article: 16 086 characters

Nathan S. Bryan considers supplements with L-arginine or L-citrulline ineffective.

  • According to Bryan, most beet products do not contain enough nitrates or nitrites to produce nitric oxide.
  • Nitrate products may work for fewer than one-third of people, and mouthwash and antacids can interfere with their use.
  • Bryan advises checking whether a product has published clinical trials in humans; the words “beetroot extract” or “kale” on the label alone do not prove that it contains enough nitrates or nitrites.
  • Henryk Różański warns that vanadium reduces nitric oxide production, which may matter to people who use “nitric oxide pumps.”

Nathan S. Bryan

Nathan S. Bryan considers products with L-arginine or L-citrulline ineffective because the body already has enough of these amino acids, or because citrulline is a by-product of nitric oxide.

„1. Products that contain L-arginine and/or L-citrulline. These products are useless. Our body makes sufficient levels of L-arginine to fully saturate the binding site of the NOS enzyme. L-citrulline is a bi-product of NO so it is not involved in NO production.”1. Products that contain L-arginine and/or L-citrulline. These products are useless. Most beet products do not contain enough nitrates or nitrites to produce nitric oxide.

„2. Beet root-based products. Most of these products do not contain sufficient nitrate or nitrite to lead to any NO formation. Most of these are “dead beat” products. 3. Nitrate based products. Several companies are selling capsules that contain potassium nitrate or sodium nitrate.”2. Beet root-based products. Nitrate products may work only for some people because the body needs the right bacteria to process them; Nathan S. Bryan estimates that they may work for fewer than one-third of people.

„As a result, a nitrate-based product may only work in less than a third of the population. Nitrate is an important nutrient, but only if you are able to metabolize it to NO. Most people cannot and that is why they are nitric oxide deficient. Nitrate does not equal nitric oxide.”As a result, a nitrate-based product may only work in less than a third of the population. Mouthwash and antacids can interfere with the use of nitrates to produce nitric oxide.

„Beet root products typically do not provide sufficient nitrate or nitrite to produce any appreciable nitric oxide. Furthermore, people taking antacids or using mouthwash are unable to utilize nitrate to make NO. Unfortunately, that is most Americans.”Beet root products typically do not provide sufficient nitrate or nitrite to produce any appreciable nitric oxide. The words “beetroot extract” or “kale” on the label alone do not prove that a product contains enough nitrates or nitrites to provide a benefit.

„Just because a product may contain beet root extract or kale on the label does not mean that it contains sufficient nitrite and/or nitrate to experience a nitric oxide benefit. So how do you know if a nitric oxide product is a true NO product that actually works?”Just because a product may contain beet root extract or kale on the label does not mean that it contains sufficient nitrite and/or nitrate to experience a nitric oxide benefit. So how do you know if a nitric oxide product is a true NO product that actually works?

Nathan S. Bryan encourages people to check whether a particular product has published clinical trials in humans.

„I encourage everyone to ask yourself 4 important questions when considering the purchase of any dietary supplement or functional nutritional product. Does the actual product have any published clinical trials in humans? Clinical trials are expensive and risky so you must have confidence that your product works. Very few companies actually put their product to the ultimate test in clinical trials.”I encourage everyone to ask yourself 4 important questions when considering the purchase of any dietary supplement or functional nutritional product. Nathan S. Bryan says that most beet products on the market have no nitric oxide activity.

„Giving more of these nutrients, like L-arginine can actually make things worse. Beets have become a popular item lately. Beets are the third least like vegetable in the world and most of the beet products on the market have no nitric oxide activity whatsoever, so there is really nothing “super” about most beet products.”Giving more of these nutrients, like L-arginine can actually make things worse. Vanadium reduces nitric oxide production, which may matter to people who use “nitric oxide pumps.”

„At the same time, it reduces nitric oxide production, which conflicts with the action profile of some anabolic supplements for athletes. People who play sports and use a “nitric oxide pump” should take this into account. Vanadium also increases the release of free radicals.”Jednocześnie zmniejsza produkcję tlenku azotu, co jest sprzeczne z profilem działania niektórych suplementów anabolicznych dla sportowców. Osoby uprawiające sport i używające "pompy tlenku azotu" powinny wziąć to pod uwagę. Wanad zwiększa też uwalnianie wolnych rodników.

A review indicates that beetroot juice may support exercise performance, but also notes the possible risks of excessive nitrate intake.

Aim of the study: Beetroot juice (BRJ) has become increasingly popular amongst athletes aiming to improve sport performances. BRJ contains high concentrations of nitrate, which can be converted into nitric oxide (NO) after consumption.

Results: Furthermore, the consumption of BRJ also has an impact on oxygen delivery to skeletal muscles, muscle efficiency, tolerance and endurance and may thus have a positive impact on sports performances. Aside from the beneficial aspects of BRJ consumption, there may also be potential health risks. Drinking BRJ may easily increase nitrate intake above the acceptable daily intake, which is known to stimulate the endogenous formation of N-nitroso compounds (NOC's), a class of compounds that is known to be carcinogenic and that may also induce several other adverse effects. Compared to studies on the beneficial effects, the amount of data and literature on the negative effects of BRJ is rather limited, and should be increased in order to perform a balanced risk assessment.

A review found that L-arginine during pregnancy was associated with, among other things, lower blood pressure and better outcomes in some high-risk pregnancies.

Aim of the study: L-Arginine (L-Arg)/Nitric Oxide (NO) system is involved in the pathophysiology of relevant Obstetric conditions. This review aims at summarizing the effects of L-Arg supplementation in pregnancy looking at safety and efficacy.

Results: Among 1028 publications, 51 studies were eligible for inclusion, 25 were performed in women, and the remnant in animals. Compared to controls/placebo, the supplementation with L-Arg reduced the development of pre-eclampsia (four studies), decreased blood pressure, and reduced the need for antihypertensive drugs in women with Hypertensive Disorders of Pregnancy (HDP, eight studies). In women carrying growth retarded fetuses, L-Arg improved fetoplacental circulation, birth weight and neonatal outcomes (five studies), while in the case of threatened preterm birth, L-Arg reduced uterine contractions (two studies). In several animal species, L-Arg supplementation in pregnancy improved reproductive performance by increasing the litter number and size. Moreover, in pre-eclamptic and metabolic syndrome experimental models, maternal hypertension and fetal growth were improved.

In older people, nitrate-rich beetroot juice lowered mean arterial pressure.

Aim of the study: This study evaluated whether changes in the oral microbiome in response to dietary nitrate and antiseptic mouthwash treatments were related to changes in nitric oxide bioavailability and vascular function.

Results: intermedia r = -0.72, P = 0.001; P. dentalis r = -0.88, P < 0.0001; Crassaminicella sp. SY095 r = -0.81, P < 0.0001). Greater blood pressure benefits from supplemental dietary nitrate in older compared to younger people are mediated primarily by the suppression of potentially harmful oral bacteria, that have been associated with ammonia production.

In a small study, beetroot juice lowered diastolic blood pressure after a single dose and improved early muscle force development after two weeks.

Aim of the study: Dietary nitrate (NO3-) supplementation can increase nitric oxide (NO) bioavailability, reduce blood pressure (BP) and improve muscle contractile function in humans. Plasma nitrite concentration (plasma [NO2-]) is the most oft-used biomarker of NO bioavailability.

Results: Biological samples were collected, resting BP was assessed, and 10 maximal voluntary isometric contractions of the knee extensors were performed at 2.5-3.5 h following supplement ingestion on day 1 and day 14. Diastolic BP was significantly lower in BR (-2 ± 3 mmHg, P = 0.03) compared to PL following acute supplementation, while the absolute rate of torque development (RTD) was significantly greater in BR at 0-30 ms (39 ± 57 N m s-1, P = 0.03) and 0-50 ms (79 ± 99 N m s-1, P = 0.02) compared to PL following two weeks supplementation. Greater WB [RSNOs] rather than plasma [NO2-] was correlated with lower diastolic BP (r = -0.68, P = 0.02) in BR compared to PL following acute supplementation, while greater skeletal muscle [NO3-] was correlated with greater RTD at 0-30 ms (r = 0.64, P=0.03) in BR compared to PL following chronic supplementation.

A meta-analysis of randomized studies found that magnesium lowered CRP and increased nitric oxide levels.

Aim of the study: Magnesium (Mg) may have several beneficial effects on human health outcomes. One hypothesized mechanism eliciting such effects is the action of Mg on serum inflammatory parameters.

Results: Generally, a low risk of bias was present. In meta-analysis, Mg supplementation significantly decreased serum C reactive protein (CRP) and increased nitric oxide (NO) levels. In descriptive findings, Mg supplementation significantly reduced plasma fibrinogen, tartrate-resistant acid phosphatase type 5, tumor necrosis factor-ligand superfamily member 13B, ST2 protein, and IL-1. In conclusion, Mg supplementation may significantly reduce different human inflammatory markers, in particular serum CRP and NO levels.

In mice, L-arginine supplementation increased time to exhaustion during treadmill running at the highest dose tested.

Aim of the study: L-arginine is a conditionally essential amino acid that serves as a substrate for nitric oxide synthase and regulates energy metabolism. While its ergogenic effects have been proposed, the mechanisms underlying its anti-fatigue properties are not fully understood.

Results: In vivo, L-arginine decreased serum LDH, CPK, NEFA, ammonia, nitric oxide, and cortisol, while enhancing blood glucose and glycogen storage in both muscle and liver. Forced swimming reduced serum lactate, whereas treadmill exercise elevated intramuscular lactate, suggesting context-dependent lactate regulation. Importantly, L-arginine did not significantly improve forced-swimming immobility time, whereas treadmill time-to-exhaustion increased at the highest dose. Antioxidant responses were improved, as reflected by normalized hepatic catalase activity. In vitro, L-arginine increased HK activity, promoted myogenin expression, and reduced ROS levels, supporting improved glucose utilization, muscle differentiation, and oxidative stress resistance.

„Magnesium oxide is a common form of magnesium found in supplements. However, due to poor absorption, magnesium oxide is more likely to have a laxative effect which may be confused with saturation of vitamin C. For better absorption, we take magnesium supplements in between meals and in divided doses. In addition to oral supplements (we like magnesium citrate), we also take Epsom salt baths twice a week.
