Cutaneous porphyria — a rare disease that can be calmed with food?

Sources: 4 · Article: 7 511 characters

Ewa Dąbrowska describes improvement in porphyria after a diet: iron and porphyrin levels fell, and skin lesions became milder or disappeared.

  • Less iron may reduce its inhibitory effect on the URO-D enzyme and improve porphyrin metabolism.
  • The diet may limit the absorption of iron from food; the possibility that it increases iron excretion in bile remains an unconfirmed explanation.
  • Ferritin and liver enzyme activity also fell in the studies, but the reported results show improvement, not a guaranteed cure for every patient.

As iron levels fall, URO-D activity rises, porphyrin production decreases, and remission may occur.

„As iron levels decrease, URO-D activity increases and porphyrin production falls, leading to clinical remission. Other treatments—chloroquine, cimetidine, interferon, plasmapheresis, and hemoperfusion—proved less effective. Because bloodletting can be burdensome and often causes anemia, and stopping treatment leads to a relapse, alternative treatments are being sought.”Wraz z obniżaniem poziomu żelaza zwiększa się aktywność URO-D i maleje produkcja porfiryn, co prowadzi do remisji klinicznej. Inne metody leczenia – chlorochina, cimetydyna, interferon, plazmafereza, hemoperfuzja – okazały się mniej skuteczne. Ponieważ krwioupusty bywają uciążliwe i często wywołują niedokrwistość, a przerwanie terapii skutkuje nawrotem choroby, poszukuje się alternatywnych metod leczenia.

In a group of 23 patients, the diet lowered iron, ferritin, aminotransferase activity, and porphyrin excretion, while skin lesions became less severe.

„We achieved a statistically significant reduction in the high aminotransferase activity, as well as a reduction in elevated iron and ferritin levels and a decrease in urinary uro- and coproporphyrin excretion. The results were better when the diet was followed for a longer period. The severity of skin lesions also decreased significantly during the diet.”Uzyskaliśmy istotne statystycznie obniżenie wysokiej aktywności aminotransferaz jak również obniżenie podwyższonego poziomu żelaza i ferytyny, a także zmniejszenie wydalania uro – i koproporfiryn w moczu. Wyniki były korzystniejsze w przypadku dłuższego okresu stosowania diety. Również nasilenie zmian skórnych znacznie zmniejszyło się w czasie diety.

After three weeks on the diet, 13 men had lower iron and ferritin levels and lower coproporphyrin excretion, and their skin lesions became less severe.

„The mean serum iron concentration was 188.6±75.7 mg/dl before the diet and 140.2±56.4 mg/dl after it; ferritin levels were 574±351 vs. 499±340 ng/ml (p = 0.04). A significant reduction in the severity of skin lesions and in urinary coproporphyrin excretion was observed.”Średnie stężenie żelaza w surowicy wynosiło 188,6±75,7 mg/dl przed dietą i 140,2±56,4 mg/dl po diecie; poziom ferrytyny: 574±351 vs. 499±340 ng/ml (p = 0,04). Zaobserwowano istotne zmniejszenie nasilenia zmian skórnych oraz wydalania koproporfiryn z moczem.

In a study of 15 patients, a diet followed for several weeks lowered previously high iron and ferritin levels, and skin lesions subsided.

„Previously elevated levels of iron, ferritin, and serum immune complexes were found to decrease. In patients whose immune complex levels were initially low, a temporary rise was observed first, followed by their complete elimination from the circulation. Skin lesions and coexisting conditions (hypertension, diabetes, and periodontal disease) went into remission, and patients with intermittent claudication were able to walk farther.”Stwierdzono obniżenie uprzednio wysokich poziomów żelaza, ferrytyny i immunokompleksów w surowicy.

The diet lowered iron, which contributes to excess porphyrin production in PCT, and may have supported the liver by providing antioxidants and limiting calories.

„Excess iron is associated with overproduction of uro- and coproporphyrins in PCT because iron inhibits URO-D. Lowering iron improves URO-D activation 9 and porphyrin metabolism. Calorie restriction also affects porphyrin metabolism: fasting may trigger an attack of acute intermittent porphyria (Welland 35, Smith 32), but in rats and in our patients with PCT, VLED reduced porphyrin excretion and accelerated the healing of skin lesions.”Nadmiar żelaza wiąże się w PCT z nadprodukcją uro- i koproporfiryn, ponieważ żelazo hamuje URO-D. Obniżenie żelaza usprawnia aktywację URO-D 9 i przemianę porfiryn. Restrykcje kaloryczne wpływają też na metabolizm porfiryn: głodówka może wywołać atak ostrej przerywanej porfirii (Welland 35, Smith 32), lecz u szczurów i naszych pacjentów z PCT VLED zmniejszyło wydalanie porfiryn i przyspieszyło gojenie zmian skórnych.

One possible explanation for the decrease in iron is reduced absorption of plant-based iron; increased excretion in bile is only a hypothesis, with no supporting studies.

„One possible reason is poorer absorption of plant-based iron due to phytates, compared with heme iron 11. Patients following the vegetable-based treatment stopped eating meat. The diet did not lower iron levels below the normal range, but restored them to a normal range, which suggests improved self-regulation of iron homeostasis at the cellular level.”Jedną z przyczyn może być gorsze wchłanianie żelaza roślinnego ze względu na fityniany w porównaniu z żelazem hemowym 11. Pacjenci w trakcie kuracji warzywnej zaprzestali spożywania mięsa. Dieta nie obniżyła poziomu żelaza poniżej normy, lecz przywróciła go do prawidłowego zakresu, co sugeruje usprawnienie samoregulacji homeostazy przemiany żelaza na poziomie komórkowym.

The diet may have increased iron excretion in bile, but the passage presents this as a possibility, not a confirmed mechanism.

„A vegetable-and-fruit diet may increase iron excretion in bile, but there are no studies confirming this mechanism. Excessive iron levels are associated with overproduction of uro- and coproporphyrins because iron inhibits uroporphyrinogen decarboxylase. Lowering iron improves porphyrin metabolism by activating the URO-D enzyme 9.”Być może dieta warzywno-owocowa zwiększa wydalanie żelaza z żółcią, ale brakuje badań potwierdzających ten mechanizm. Nadmierny poziom żelaza wiąże się z nadprodukcją uro- i koproporfiryn, gdyż żelazo hamuje uroporfirynogenu dekarboksylazę. Obniżenie żelaza poprawia przemianę porfiryn poprzez aktywację enzymu URO-D 9.

Removing about 3.5 g of iron requires bloodletting; at most, the diet may support a small amount of iron excretion in bile.

„Only bloodletting plays an active role in removing excess iron (about 3.5 g), although VLED may stimulate a small amount of iron excretion in bile 12. Excess iron is associated with overproduction of uro- and coproporphyrins in PCT because iron inhibits URO-D. Lowering iron improves URO-D activation 9 and porphyrin metabolism.”W usunięciu nadmiaru żelaza (ok. 3,5 g) czynną rolę odgrywają wyłącznie upusty krwi, lecz VLED może stymulować niewielkie wydalanie żelaza z żółcią 12. Nadmiar żelaza wiąże się w PCT z nadprodukcją uro- i koproporfiryn, ponieważ żelazo hamuje URO-D. Obniżenie żelaza usprawnia aktywację URO-D 9 i przemianę porfiryn.

The diet provides antioxidants, and calorie restriction may reduce the formation of free radicals and support the mechanisms that remove them.

„A vegetable-and-fruit diet combines several effects that may protect the liver from free-radical reactions and prevent an increase in transaminase activity because: the diet normalizes the elevated iron (an oxidant) levels in patients with PCT, as our studies have shown; it provides antioxidant vitamins 34 (several times more vitamin C and carotene than recommended) as well as flavonoids, which neutralize free radicals directly 2 or indirectly by chelating iron and copper ions 27;”Kuracja dietą warzywno-owocową łączy wielokierunkowe działania, które mogą chronić wątrobę przed reakcjami wolnorodnikowymi i zapobiegać wzrostowi aktywności transaminaz, gdyż: dieta normalizuje podwyższony poziom żelaza (oksydanta) u chorych z PCT, jak wynika z naszych badań; jest źródłem antyoksydacyjnych witamin 34 (dostarcza kilkakrotnie więcej witaminy C i karotenu niż w zaleceniach) oraz flawonoidów neutralizujących wolne rodniki bezpośrednio 2 lub pośrednio przez chelatowanie jonów żelaza i miedzi 27;

The diet significantly lowered circulating immune complexes and was associated with improved clinical condition.

„A vegetable-and-fruit diet significantly lowers the level of circulating immune complexes, helping to normalize immune parameters and improve the clinical condition of patients with porphyria cutanea tarda.”Dieta warzywno-owocowa znacząco obniża poziom krążących kompleksów immunologicznych, przyczyniając się do normalizacji parametrów immunologicznych i poprawy stanu klinicznego u pacjentów z porfirią późną skórną.

Iron inhibits URO-D, and lowering it increases the enzyme’s activity and may lead to temporary remission.

„Because iron inhibits URO-D activity 22, current treatment for the disease focuses mainly on reducing iron stores through frequent bloodletting 3,17 or infusions of deferoxamine (iron chelation) 33. As iron levels decrease, URO-D activity increases and porphyrin production falls, which may lead to temporary clinical remission.”Ponieważ żelazo hamuje aktywność URO-D22 dlatego też aktualna terapia choroby koncentruje się głównie na obniżaniu zapasów żelaza przez często powtarzane upusty krwi 3,17 lub wlewy desferioksaminy (chelatowanie żelaza)33 Wraz z obniżaniem poziomu żelaza – zwiększa się aktywność URO-D, obniża produkcja porfiryn, co może doprowadzić do czasowej remisji klinicznej.
