Low testosterone in men — what do doctors say, and how can you raise it naturally?

Sources: 13 · Article: 15 071 characters

Jerzy Zięba claims that selenium may raise testosterone in men whose levels are deficient.

  • Igor Dogocki lists exercise, maintaining a healthy body weight, herbs, and avoiding alcohol as ways to raise testosterone and estrogen levels.
  • Henryk Różański says that tribulus may raise natural testosterone at an appropriate dose, and that its effects depend on the composition of the preparation.

What the authors write

Jerzy Zięba

Selenium supplementation may raise testosterone in men whose levels are deficient.

„This therefore increases both the chances of conception and the child’s future health. Most antioxidants, such as Coenzyme Q10, vitamin C, or L-carnitine, mainly affect one or two of the parameters listed above. Selenium also increases testosterone levels in men who have a deficiency of this hormone.”Zwiększa zatem zarówno szanse poczęcia jak również przyszłą zdrowotność dziecka.

Exercise, maintaining an appropriate body weight, herbs, and avoiding alcohol are listed as ways to raise testosterone and estrogen levels.

„Testosterone/estrogen levels can be raised through exercise, maintaining a body weight appropriate for one’s height, consuming herbs, and avoiding alcohol. Other causes of gum disease include: bleeding in the front section, heavy metal poisoning (e.g.”Poziom testosteronu/estrogenu można podnieść dzięki ćwiczeniom, utrzymaniu masy ciała odpowiedniej dla wzrostu, spożywanie ziół oraz unikanie alkoholu. Do innych przyczyn chorób dziąseł możemy zaliczyć: krwawienie w odcinku przednim zatrucie metalami ciężkimi (np.

Tribulus may raise natural testosterone at an appropriate dose, but its effects depend on the composition of the preparation.

„At an appropriate dose, it raises natural, endogenous testosterone levels, but at the same time blocks androgen receptors in the prostate gland. This may reduce inflammation and swelling of the prostate, make urination easier, and reduce pain during urination. Tribulus, like black cumin, fenugreek, yucca, and smilax, increases free testosterone levels in the blood. The effects of a tribulus preparation depend on the dose, the content of steroidal saponins, and the ratio of phytosterols to saponins and flavonoids.”W odpowiedniej dawce podnosi poziom naturalnego, endogennego testosteronu, ale równocześnie blokuje receptory androgenowe w gruczole krokowym, przez co może zmniejszać stan zapalny i obrzęk prostaty, ułatwiać oddawanie moczu i zmniejszać ból podczas mikcji.

Selenium can be obtained from fish, seafood, and Brazil nuts, but its amount in plant foods varies greatly.

„Brazil nuts are an important source of this mineral. However, it is difficult to estimate how much selenium they contain because soil concentrations vary widely. Nuts from Brazil and Bolivia—the ones most commonly available on the market—do not contain much selenium, while nuts from Peru or Colombia do.”Istotnym źródłem tego minerału są orzechy brazylijskie.

Nettle extracts lower SHBG and inhibit aromatase, but the passage does not explicitly say that they raise testosterone.

„Water-alcohol extracts from nettle rhizomes and roots lower levels of SHBG, the protein that binds sex hormones: androgens and estrogens. They also inhibit aromatase and slightly reduce the activity of 5-alpha-reductase (this enzyme breaks down testosterone into 5-alpha-dihydrotestosterone, the androgen that acts at the cellular level).”Wodno-alkoholowe wyciągi z kłączy i korzeni pokrzywy obniżają stężenie SHBG, czyli globuliny wiążącej hormony płciowe: androgeny i estrogeny.

Hops lower free testosterone in men.

„In women, it raises estrogen levels; in men, it lowers free testosterone levels. It also lowers progesterone. It should be emphasized that hops do not reduce male fertility (the ability to conceive), but only excessive sexual drive. Probably by reducing the effects of testosterone and adrenaline, they also lower readiness to fight and defend oneself and reduce aggression.”U kobiet podwyższa poziom estrogenów, u mężczyzn obniża poziom testosteronu wolnego. Wywiera też wpływ obniżający na progesteron. Należy podkreślić, że chmiel nie obniża płodności (zdolności zapłodnienia) mężczyzn, a jedynie nadmierny popęd płciowy. Prawdopodobnie przez niwelowanie działania testosteronu i adrenaliny obniża też gotowość do walki i obrony oraz zmniejsza agresję.
Nathan S. Bryan

Nathan S. Bryan describes low testosterone in men as a global phenomenon.

„We have a global pandemic of low testosterone in men and a plethora of hormone (estrogen and progesterone) deficiencies or imbalances in women. There are an alarming number of patients with heart arrhythmias such as atrial fibrillation. The science is clear on the mechanism of these side effects.”We have a global pandemic on low testosterone in men and a plethora of hormone (estrogen and progesterone) deficiencies or imbalances in women. The review indicates that lifestyle changes can raise the body’s own testosterone production.

Aim of the study: Testosterone deficiency is defined as a total testosterone level <300 ng/dL confirmed on two early morning lab draws. Testosterone therapy has historically been offered to men with symptomatic testosterone deficiency in the form of injections, gels, or pellets.

Results: However, this medication is expensive and requires patients to administer their own injections. Over-the-counter herbal supplements and designer steroids remain available, although they are poorly studied and are associated with the potential for abuse as well as increased liver and cardiovascular risks. This review aims to discuss existing alternatives to traditional testosterone therapy, including the effectiveness, safety, and side effects of these options. The authors suggest that the SERM clomiphene citrate (CC) holds the greatest promise as a non-testosterone treatment option for testosterone deficiency.

In men with obesity, significant weight loss can reverse the associated reduction in testosterone.

Aim of the study: Pathologic hypogonadism occurs when serum testosterone is significantly and persistently reduced by irreversible organic (structural, genetic) disorders of the hypothalamic-pituitary-testicular axis. Men with pathologic hypogonadism require lifelong testosterone replacement.

Results: Herein we demonstrate how clinically significant weight loss substantially reverses obesity-related reductions in serum testosterone and ameliorates nonspecific symptoms resembling, but not due to, androgen deficiency. The important reversible steps include weight reduction and optimizing management of type 2 diabetes mellitus, obstructive sleep apnea, depression, and other obesity-related comorbidities as well as rationalizing concomitant drug regimens. In the absence of pathological hypogonadism, testosterone treatment is less effective than a diet and lifestyle intervention to rectify the reversible conditions responsible for the nonspecific symptoms and associated reduced serum testosterone concentrations observed in men with obesity. As such, testosterone treatment is not indicated, and unwarranted off-label testosterone treatment can lead to adverse effects such as infertility, elevated hematocrit requiring venesection, a prothrombotic state, and testosterone dependence.

Significant weight loss can reverse obesity-related hypogonadism.

Aim of the study: The single most significant risk factor for testosterone deficiency in men is obesity. The pathophysiological mechanisms involved in male obesity-related secondary hypogonadism are highly complex.

Results: Lifestyle measures are the cornerstone of management, as they can potentially improve androgen deficiency symptoms regardless of their effect on testosterone levels. In selected patients, bariatric surgery can reverse obesity-induced hypogonadism. If these measures fail to relieve symptoms and normalize testosterone levels, testosterone replacement therapy could be started in appropriately selected men. Aromatase inhibitors and selective estrogen receptor modulators are not recommended because there is not enough consistent evidence from clinical trials.

Weight loss and better management of coexisting conditions can reverse functional suppression of the hormonal axis.

Aim of the study: Obesity and dysglycemia (comprising insulin resistance, the metabolic syndrome and type 2 diabetes), that is diabesity, are associated with reduced circulating testosterone and, in some men, clinical features consistent with androgen deficiency. To review the metabolic impact of late-onset hypogonadism.

Results: Obesity is one of the strongest modifiable risk factors for late-onset hypogonadism, and coexisting diabetes leads to further hypothalamic-pituitary-testicular axis suppression. This suppression is functional and therefore potentially reversible, and occurs predominantly at the level of the hypothalamus. While definitive mechanistic data are lacking, the evidence suggests that hypothalamic-pituitary-testicular axis suppression is mediated by dysregulation of pro-inflammatory cytokines leading to hypothalamic inflammation. Dysregulation of central leptin and insulin signaling may also contribute. In contrast, recent data challenge the idea that excess estradiol is a major contributor to hypothalamic-pituitary-testicular axis suppression. Instead, relatively weak estradiol signaling may contribute to metabolic dysregulation in men with diabesity.

In men with obesity, lifestyle changes are usually associated with a small increase in testosterone.

Aim of the study: Obesity, which is increasing in prevalence globally, is the clinical condition most strongly associated with lowered testosterone concentrations in men and is one of the strongest predictors of receiving testosterone treatment.

Results: Short-term studies have shown that testosterone treatment in carefully selected obese men may have modest benefits for symptoms of androgen deficiency and body composition, even in addition to diet alone. However, larger, longer-term RCTs designed to assess outcomes that matter to patients and potential risks are needed. Until such trials are available, testosterone treatment cannot be routinely recommended for men with obesity-associated nonclassical hypogonadism. Lifestyle measures or, where indicated, bariatric surgery to achieve weight loss, and optimization of comorbidities remain first-line approaches.

The review indicates that weight loss and improved sleep may increase testosterone levels.

Aim of the study: Over the past decade, there have been concerns about the safety of testosterone therapy (TTh) in hypogonadal men. Several concerns have centered on the use of TTh and its potential link to cardiovascular (CV) events, prostate cancer, and benign prostatic hyperplasia (BPH).

Results: Most patients treated with TTh today are treated off-label. Low serum testosterone levels have been associated with increased CV events. Currently, there is inconclusive evidence to support the claim that TTh increases the risk of CV events. There is no evidence linking TTh to the development of prostate cancer or worsening of BPH symptoms. Finally, lifestyle modification, such as losing weight and improving sleep, can improve serum testosterone levels in hypogonadal men. Clinicians prescribing testosterone should be aware of the current controversies associated with TTh. The current literature does not suggest that there is a significant risk of prostate cancer, worsening BPH symptoms, or CV events with TTh. However, more studies, including randomized placebo-controlled trials, are needed. Finally, patients should be appropriately counseled about the indications for TTh and the benefits of lifestyle modification before starting TTh.

In men with metabolic disorders, studies of GLP-1 drugs have reported an increase in total testosterone, but it is unclear how much of this was due to the treatment itself and how much to weight loss.

Aim of the study: Glucagon-like peptide-1 (GLP-1) receptor agonists and the dual glucose-dependent insulinotropic polypeptide (GIP)/GLP-1 receptor agonist tirzepatide are increasingly used to treat obesity and type 2 diabetes in men of reproductive age, but practical guidance for counseling men planning fatherhood remains limited. This structured narrative review distinguishes endocrine, semen, sexual-function, and clinical fertility outcomes.

Results: Evidence on paternal preconception exposure is insufficient, and pregnancy-related label precautions do not by themselves establish risk from paternal exposure. No eligible controlled tirzepatide-specific study assessing semen or clinical fertility outcomes was identified by this search. These therapies should be used for established metabolic indications, not as male infertility treatments. The proposed framework was developed by the authors, is informed by evidence, has not been validated, and is intended for individualized counseling and prospective evaluation.
