Igor Dogocki

Probiotics may restore the balance of the oral microbiome and limit harmful bacteria, which may reduce the risk of tooth decay and gum disease.

„When it comes to the mouth, probiotics show potential for restoring the balance of the oral microbiome and reducing the risk of dental diseases; recent studies suggest they may also reduce the risk of gum disease. These beneficial bacteria can change the ecosystem of our mouth.”Jeśli chodzi o jamę ustną, probiotyki wykazują potencjał w przywracaniu równowagi mikrobiomu jamy ustnej i zmniejszają ryzyko chorób zębów a z ostatnich badań wynika, iż również dziąseł. Te korzystne bakterie mogą zmieniać ekosystem naszej jamy ustnej.
Igor Dogocki

Probiotic strains may inhibit plaque and disease-causing bacteria and support anti-inflammatory effects.

„Lactobacillus strains have probiotic effects. Their mechanisms include inhibiting the formation of dental plaque biofilm, suppressing the growth of pathogenic bacteria by producing antimicrobial compounds, and indirectly interacting with the immune system to modulate the host’s immune response.”Szczepy Lactobacillus wykazują działanie probiotyczne, a mechanizmy działania obejmują hamowanie tworzenia biofilmu płytki nazębnej, hamowanie rozwoju bakterii patogennych poprzez produkcję związków przeciwdrobnoustrojowych oraz pośrednią interakcję z układem immunologicznym, modulując ą odpowiedź immunologiczną gospodarza.
Igor Dogocki

Limosilactobacillus reuteri and Bifidobacterium are identified as bacteria associated with improved gum health.

„We already know that bacteria such as Porphyromonas gingivalis, Tannerella forsythia and Treponema denticola are very common in periodontitis, while others, such as Limosilactobacillus reuteri and Bifidobacterium, contribute to better gum health.”Wiemy już, że b akterie, takie jak Porphyromonas gingivalis, Tannerella forsythia i Treponema denticola, są bardzo rozpowszechnione przy zapaleniu przyzębia, podczas gdy inne, takie jak Limosilactobacillus reuteri i Bifidobacterium, przyczyniają się do poprawy zdrowia dziąseł.

Igor Dogocki warns that harsh antiseptic mouthwashes may disrupt the microbiome, and that long-term use may do more harm than good.

„Although some antimicrobial agents may be needed to treat the acute phase of a disease, long-term use and side effects may do more harm than good. Let’s look, for example, at the most common ingredients in antibacterial mouthwashes: Alcohol—it has an antimicrobial effect and does not distinguish between good and bad bacteria.”Chociaż niektóre środki przeciwdrobnoustrojowe mogą być konieczne do leczenia ostrej fazy choroby, długoterminowe stosowanie i skutki uboczne mogą być bardziej szkodliwe niż korzyści. Przyjrzyjmy się na przykład najczęstszym składnikom płukanek antybakteryjnych do ust : Alkohol – ma działanie przeciwdrobnoustrojowe i nie odróżnia dobrych i złych bakterii.
Igor Dogocki

An alcohol-based mouthwash may kill both good and bad bacteria and reduce microbiome diversity.

„Let’s look, for example, at the most common ingredients in antibacterial mouthwashes: Alcohol—it has an antimicrobial effect and does not distinguish between good and bad bacteria. This means that alcohol-based mouthwash simply reduces microbiome diversity. Hydrogen peroxide—it is a cleaning agent that often runs into the mouth.”Przyjrzyjmy się na przykład najczęstszym składnikom płukanek antybakteryjnych do ust : Alkohol – ma działanie przeciwdrobnoustrojowe i nie odróżnia dobrych i złych bakterii. Oznacza to, że płyn do płukania ust na bazie alkoholu po prostu zabija różnorodność mikrobiomu Nadtlenek wodoru – to środek czyszczący, który często spływa do ust.

Strains used in dental products include Lactobacillus acidophilus, Lactobacillus reuteri and Streptococcus salivarius.

„The most common probiotic strains used in dental products include Lactobacillus acidophilus, Lactobacillus reuteri and Streptococcus salivarius. These strains encourage the growth of beneficial bacteria, which may replace harmful bacteria in the mouth.”Do najpowszechniejszych szczepów probiotycznych stosowanych w produktach dentystycznych, należą: Lactobacillus acidophilus, Lactobacillus reuteri i Streptococcus salivarius. Szczepy te sprzyjają rozwojowi pożytecznych bakterii, które mogą zastąpić te szkodliwe, występujące w jamie ustnej.
Igor Dogocki

Igor Dogocki recommends considering oral probiotics, including Streptococcus salivarius.

„Optimize your gut and oral microbiomes. You can take prebiotics, probiotics (for the mouth, Streptococcus salivarius) and butyric acid supplements. Maintain a good vitamin D level (always take it with vitamin K2), supplement with liposomal vitamin C and collagen, and, if your gums bleed, also take Coenzyme Q.”Zoptymalizuj swój mikrobiom jelit oraz jamy ustnej. Możesz przyjmować prebiotyki, probiotyki (w jamie ustnej Streptococcus salivarius) oraz preparaty kwasu masłowego. Dbaj o dobry poziom witaminy D (przyjmowana zawsze z witaminą K2), suplementuj liposomalną witaminę C, kolagen oraz w razie krwawienia dziąseł dodatkowo Coenzym Q.

Probiotics can be used on their own or with conventional periodontal treatment as part of oral care and tooth-decay prevention.

„Probiotics, used alone or alongside conventional periodontal treatment, are likely to become a mainstay of oral care in the coming years. As eliminating all microbes from the mouth is rapidly becoming an outdated approach to oral hygiene, the benefits of using health-promoting probiotic bacteria as part of gum treatment and tooth-decay prevention are widely recognized.”Probiotyki, stosowane samodzielnie lub w połączeniu z klasycznym leczeniem periodontologicznym, najprawdopodobniej staną się podstawą pielęgnacji jamy ustnej w nadchodzących latach.
Igor Dogocki

Oral supplements containing Limosilactobacillus reuteri and a blend of several probiotic strains are mentioned.

„Manufacturer’s website Gum PerioBalance: GUM PerioBalance is an advanced dietary supplement for oral use. It contains a patented bacterium of human origin called Limosilactobacillus reuteri Prodentis® (DSM 17938 and ATCC PTA5289), previously known as Lactobacillus reuteri Prodentis®.”strona producenta Gum PerioBalance GUM PerioBalance to zaawansowany suplement diety do stosowania w jamie ustnej zawierający opatentowaną bakterię pochodzenia ludzkiego o nazwie Limosilactobacillus reuteri Prodentis® (DSM 17938 i ATCC PTA5289) znaną wcześniej jako Lactobacillus reuteri Prodentis®.

Henryk Różański recommends a rinse made with plant extracts and oil fractions, which he says has anti-inflammatory effects and limits the growth of microorganisms.

„It is based on plant extracts and oil fractions. The author of this product knew what he was doing. When diluted with water, it makes an excellent rinse. You can also brush your teeth and gums with it undiluted after brushing, as it works very well. It clears up all inflammation, inhibits the growth of microorganisms, promotes the healing of erosions, stops gum bleeding and discharge, and removes bad breath.”Oparty został na ekstraktach z roślin i frakcjach olejkowych. Autor tego preparatu znał sie na rzeczy. Po rozcieńczeniu z wodą świetna płukanka. Można nim również (nierozcieńczonym) szczotkować zęby i dziąsła po umyciu, działa bowiem doskonale. Usuwa wszelkie stany zapalne, hamuje rozwój drobnoustrojów, wzmaga gojenie nadżerek, hamuje krwawienia i wysięki z dziąseł, usuwa przykry zapach.
Igor Dogocki

Probiotic bacteria are presented as helping protect against tooth decay and gum disease.

„We already know that probiotic bacteria, which protect against tooth decay and gum disease, as well as other bacteria found in the mouth, migrate every day—among other routes, by being swallowed—to almost every part of the body, including the heart, liver, kidneys and brain. It has also been found that populations with no dental diseases have oral microbiomes with much greater diversity.”Wiemy już, że bakterie probiotyczne, które chronią przed próchnicą i chorobami dziąseł a także inne bakterie występujące w jamie ustnej migrują codziennie m.i.n. połykane prawie do wszystkich części ciała, w tym do serca wątroby, nerek i mózgu. Okazało się również, że społeczeństwa, w których nie miały chorych chorób zębów, mają mikrobiomy jamy ustnej o znacznie większej różnorodności.

A meta-analysis found that probiotics reduce bad breath and levels of volatile sulfur compounds in the short term.

Aim of the study: Halitosis is defined as a foul odour emitted from the oral cavity. Many interventions have been used to control halitosis from mouthwashes to chewing gums. Probiotics have been reported as an alternative method to alleviate halitosis. The present study aimed to investigate the effect of probiotics on halitosis from a time perspective.

Results: The standardised mean difference (SMD) and 95% CI were calculated to synthesise data. The data were subgrouped and analysed in the short term (≤4 weeks) and long term (>4 weeks) based on the follow-up time. Seven articles were included in this meta-analysis. The primary outcomes, OLP scores (SMD=-0.58; 95% CI -0.87 to -0.30, p<0.0001) and VSC levels (SMD=-0.26; 95% CI -0.51 to -0.01, p=0.04), both decreased significantly in the probiotics group compared with the placebo group in the short term. However, a significant reduction was observed only in OLP scores (SMD=-0.45; 95% CI -0.85 to -0.04, p=0.03) in the long term. No significant differences were observed in secondary outcomes. There was no evidence of publication bias. The leave-one-out analysis confirmed that the pooled estimate was stable.

The review found that most studies reported a reduction in sulfur compounds, and some also reported improved breath odor scores after probiotic use.

Aim of the study: Halitosis, characterized by volatile sulfur compound (VSC) production, is a prevalent oral health concern affecting 31.8% of the global population. Conventional treatments such as chlorhexidine-based mouthwashes offer temporary relief, but probiotics have emerged as a promising biological therapy by modulating oral microbiota.

Results: Six RCTs (n = 360 participants) met the inclusion criteria. Five studies demonstrated significant VSC reduction after probiotic intervention (P < 0.05). Three studies reported OLT score improvements, particularly with Streptococcus salivarius K12 and Weissella cibaria. Four studies confirmed microbiome alterations, with the effects of probiotics persisting post-treatment. No serious adverse effects were reported. Probiotics significantly reduce VSC levels, improve OLT scores, and modulate the growth of oral microbiota. However, the heterogeneity of studies and limited long-term follow-up hinder clinical translation. Larger, standardized trials are essential for establishing the clinical efficacy. Probiotics present a safe adjunct therapy for halitosis management. Future research should focus on longitudinal studies, specific strain efficacy, and microbiome-targeted interventions.

The review found that probiotics reduced the number of bacteria that cause tooth decay and were at least as effective as chlorhexidine in improving plaque and gum indices.

Aim of the study: Periodontal diseases and dental caries are the two most common dental diseases caused by the dental plaque. The aim of the present study was to review the clinical efficacy of probiotics for oral health in randomized controlled trials.

Results: A total of 24 clinical trials were reviewed, including 1612 participants receiving either probiotics or mouth treatments. The results of this review indicated that individuals receiving probiotic products have a significant reduction (65% reduction; p < 0.05) in the count of Streptococcus mutants in their mouths. It was also found that probiotic products were more effective or equal in effect compared to chlorhexidine in reducing oral pathogens, gingival index, and plaque index scores. On the other hand, the consumption of xylitol mouthwash was shown to cause an improvement in salivary parameters. Considering their safety and effectiveness, the use of probiotic products, such as kefir and mouthwashes, has been recommended against cariogenic bacteria and periodontal diseases. Probiotics are considered a safe alternative to conventional therapies, such as chlorhexidine and fluoride. Co-administration of chlorhexidine, fluoride, and probiotics seems to be a perfect package.

In the study, probiotic mouthwash reduced plaque and bleeding similarly to chlorhexidine mouthwash.

Aim of the study: Chemical plaque control with mouthwashes as an adjunct to mechanical plaque control with a toothbrush and dental floss has been considered an effective method for controlling gingivitis. The anti-inflammatory effects of chemical plaque control benefit the oral tissues by reducing inflammation and bleeding.

Results: All groups showed a significant decrease in the MPI and BOIB scores. The results were similar in patients who used a probiotic mouthwash and those who used a CHX mouthwash. A comparable change in the mean scores was observed among the herbal and povidone-iodine groups from baseline to day 28. In the treatment of chronic gingivitis patients,a probiotic mouthwash was nearly as effective as CHX in reducing the plaque and bleeding scores. It showed better results in all clinical parameters than herbal and povidone-iodine mouthwashes. Using a mouthwash along with routine tooth brushing can help in treating gingivitis and slow the progression of the periodontal disease.

Probiotic mouthwash significantly reduced the number of Streptococcus mutans bacteria in children and older adults.

Aim of the study: Chlorhexidine is considered the most potent chemotherapeutic agent against Streptococcus mutans. However, its side effects due to prolonged use, indicates need for alternatives. The study intended to assess and compare antimicrobial efficacies of probiotic, xylitol and chlorhexidine mouth rinses in children and elderly.

Results: Significant reduction in Streptococcus mutans counts were observed in both children and elderly (Chlorhexidine: mean difference = 3.11 log10CFU/g, p = 0.022, Xylitol: mean difference = 0.93 log10CFU/g, p = 0.046, Probiotic: mean difference = 1.91 log10CFU/g, p = 0.023 in children); (Chlorhexidine: mean difference = 2.23 log10CFU/g, p = 0.004, Xylitol: mean difference = 1.39 log10CFU/g, p = 0.009, Probiotic: mean difference = 1.61 log10CFU/g, p = 0.018 in elderly). Intergroup comparison showed no significant difference. Antimicrobial efficacy of xylitol and probiotic mouth rinses were comparable to that of chlorhexidine in both children and elderly. Probiotics could potentially be more efficacious than xylitol among children.

Mouthwash containing probiotic Lactobacillus brevis eliminated Pseudomonas from the mouth and reduced the number of ulcers.

Aim of the study: A selenium (Se) enriched Lactobacillus brevis was applied as mouthwash to relieve mouth ulcers and regulate oral pathogens. The persistence of four frequent oral pathogens (Pseudomonas, Klebsiella, Proteus, and Enterobacter) and the quantity of mouth lesions were to be measured after exposure to the above probiotic. Susceptibility of the pathogens was also examined.

Results: Klebsiella and Pseudomonas were the most frequent initial pathogens, from which Pseudomonas were totally omitted from the probiotic group. The Se-probiotic existed in the mouth cavity of the test group six hours after the last dose. In the probiotic group, the number of oral lesions were significantly reduced. Clearance of Pseudomonas from mouth cavity and significant decrease in the quantity of mouth ulcers are considerable achievements. Hence, the applied Se-supplemented mouthwash exhibited promising capability to be considered as a potential oral hygiene formulation.

Combining probiotics with ozonated olive oil was associated with the greatest improvement in the dental plaque index in people with periodontitis.

Aim of the study: Background: Researchers are now focusing on new and less invasive therapies to improve the domiciliary maintenance phase of periodontitis. Ozonated olive oil as an alternative to common local antiseptics and the assumption of probiotics to maintain a eubiotic oral microbiome show promising results.

Results: Also, the FMBS percentages were significantly decreased (p < 0.0001): -15%, -20%, and -49% observed in group A, group B, and group C, respectively. The mean PPD showed significant differences (p < 0.0001): -0.10 mm, -0.40 mm, and -1.10 mm observed in group A, group B, and group C, respectively. Overall, group C showed the best improvement among the considered clinical indexes. Conclusions: The findings of this clinical trial support the use of a combined regime of the antimicrobial and anti-inflammatory effects of ozonated olive oil and the modulation of the oral microbiome of probiotic supplements as an adjunctive domiciliary strategy for patients affected by stage I and II periodontitis.
