Kava-kava — an island drink for anxiety and stress, or a danger to the liver?

Sources: 11 · Article: 19 506 characters

Henryk Różański lists kava-kava as a remedy for anxiety, insomnia, fatigue, and muscle spasms and tension.

  • Henryk Różański also mentions inflammation and infections of the urinary and reproductive systems, as well as some fungal infections and seizures.
  • In the past, it was used in preparations for infections of the urinary and reproductive systems, including gonorrhea. A combination with sandalwood oil was also described as relieving pain and spasms in the urinary tract.
  • Henryk Różański warns that its effects may vary because the raw material available is sometimes not standardized.

According to Henryk Różański, kava-kava preparations are said to help relax the muscles, allow massage in cases of severe spasms, and induce hypnosis or meditation.

„Preparations of Piper methysticum make it easier to induce hypnosis and meditation in patients. Applied by rubbing in and taken orally, they help relax the muscles and carry out therapeutic massages in cases of severe spasticity. Kavalactones enhance the effects of greater celandine (Chelidonium), viper's bugloss (Echium), Corydalis (Corydalis), and similar alkaloid-containing plants (pain-relieving, cough-suppressing, and sedative effects).”Preparaty z Piper methysticum ułatwiają wprowadzenie pacjentów w hipnozę i do medytacji. Wcierane i podawane doustnie ułatwiają relaksację mięśni i przeprowadzenie masaży leczniczych przy silnej spastyczności. Kawalaktony potęgują działanie glistnika – Chelidonium, żmijowca – Echium, kokoryczy – Corydalis i podobnych alkaloidowych roślin (efekt przeciwbólowy i przeciwkaszlowy, sedatywny).

Kava-kava was formerly used to treat diseases of the urinary and reproductive systems, and is now associated with the treatment of nervous system disorders.

„Today, kava pepper — Piper methysticum G. Forster, better known as Kava-Kava — is associated with a plant useful in treating diseases of the nervous system. Before the war, however, it was used to treat diseases of the urinary and reproductive systems. Kava-Kava belongs to the pepper family, Piperaceae.”Obecnie pieprz metystynowy – Piper methysticum G. Forster, znany bardziej pod nazwą Kava-Kava jest kojarzony z rośliną przydatną w leczeniu chorób układu nerwowego. Tymczasem przed wojną miał zastosowanie w terapii chorób układu moczowo-płciowego. Kava-Kava należy do rodziny pieprzowatych – Piperaceae.

In the past, kava-kava preparations were used for infections of the urinary and reproductive systems, including gonorrhea.

„Gonocin (Temmler-Werke, Vereinigte Chem. Fabriken, Berlin-Johannisthal) — tablets containing Kava-Kava extract, Pichi-Pichi extract, bearberry leaf, salol, and hop glands (lupulin): 1–2 tablets 3 times daily for infections of the urinary and reproductive systems, including gonorrhea (antigonorrheal treatment).”Gonocin (Temmler-Werke, Vereinigte Chem.

Another former preparation containing kava-kava resin was used to disinfect the kidneys and bladder and to treat gonorrhea.

„Gonion — capsules (F. Trenka, Chem.-Pharm Fabrik, Vienna): Kava-Kava resin 0.02 g, Paronychia extract 0.05 g, bearberry leaf extract 0.05, phenyl salicylate (salol) 0.1 g: 2–4 capsules twice daily as a disinfectant for the kidneys and urinary bladder and against gonorrhea.”Gonion – kapsułki (F. Trenka, Chem.-Pharm Fabrik, Wien): żywica Kava-Kava 0,02 g, ekstrakt z połonicznika 0,05 g, ekstrakt z liści mącznicy 0,05, salicylan fenylu (salol) 0,1 g: 2-4 kapsułki 2 razy dziennie jako środek odkażający nerki, pęcherz moczowy i przeciwko rzeżączce.
Henryk Różański

Kava-kava resin was also combined with other ingredients in a preparation used against gonorrhea.

„6. Gonoform-Tabletten (Dr Hugo Remmler A.-G., Berlin): hexamethylenotetramine (that is, urotropine), phenyl salicylate, Kava-Kava resin — as an antigonorrheal treatment. See a mention of another preparation containing Kava-Kava: https://rozanski.li/?p=1163”6. Henryk Różański

A combination of kava-kava and sandalwood oil was described as inhibiting bacteria and relieving pain and spasms in the urinary tract in cases of gonorrhea.

„On the one hand, the preparation inhibited bacterial growth and killed bacteria; on the other, it relieved pain (an anesthetic and pain-relieving effect) and spasticity (spasms) in the urinary tract during gonorrhea. Treatment lasted 1 month.”Z jednej strony preparat hamował rozwój bakterii i działał bakteriobójczo, z drugiej natomiast znosił dolegliwości bólowe (efekt znieczulenia, dz. przeciwbólowego) oraz spastyczności (skurcze) w drogach moczowych w przebiegu rzeżączki. Leczenie trwało 1 miesiąc.
Henryk Różański

The effects of the raw material may vary because the level of active ingredients depends on its quality, and the material available commercially is often not standardized.

„Raw material available on the international market (it is not approved in Poland) is often not standardized for kavalactone content, which means that its pharmacological activity varies. Sometimes it is so weak that patients and doctors doubt its effectiveness. If we expect Piper methysticum to have a medicinal effect, we need to use raw material of genuinely high quality.”Surowiec dostępny w handlu zagranicznym (w Polsce nie jest dopuszczony) często nie jest standaryzowany na zawartość kavalaktonów, przez co wykazuje różną aktywność farmakologiczną, niekiedy tak słabą, że pacjenci i lekarze wątpią w jej efektywność. Jeśli oczekujemy od Piper methysticum działania leczniczego trzeba zastosować surowiec naprawdę wysokiej jakości.

A meta-analysis found that kava may reduce anxiety symptoms, although it probably does not help with generalized anxiety disorder.

Aim of the study: Anxiety disorder is a common psychiatric illness. Medicinal herbs have become a field of interest in the treatment of anxiety. This study aimed to evaluate and compare the efficacy and acceptability of all possible medicinal herbs for the treatment of anxiety.

Results: A total of 29 trials were reviewed, comparing 12 medicinal herbs. Silexan (mean difference [MD]: -3.84, 95% credible interval [CrI]: -6.31 to -1.34) displayed a significant effect on anxiety, and possibly benefitted the treatment of depression (standard mean difference [SMD]: -0.37, 95% confidence interval [CI]: -0.53 to -0.20) and insomnia (SMD: -0.48, 95% CI: -0.76 to -0.21). Kava was found to be an effective anxiolytic (MD: -2.46, 95% CrI: -4.47 to -0.32) but possibly ineffective in patients with generalized anxiety disorder (MD: -0.17, 95% CrI: -2.55 to -1.97). Ginkgo biloba (MD: -4.63, 95% CrI: -9.01 to -0.23) and Withania somnifera (MD: -4.90, 95% CrI: -9.70 to -0.17) were efficacious, as measured by HAMA scores but the trials were limited by their small sample sizes.

A review of studies found that kava extract may reduce anxiety more effectively than a placebo.

Aim of the study: Synthetic anxiolytic drugs are effective for treating anxiety, but they are burdened with adverse effects. Constraints on resources and time often render therapies such as psychologic interventions impracticable.

Results: Superiority of kava extract over placebo was suggested by all seven reviewed trials. The meta-analysis of three trials suggests a significant difference in the reduction of the total score on the Hamilton Rating Scale for Anxiety in favor of kava extract (weighted mean difference, 9.69; 95% confidence interval, 3.54-15.83). These data imply that kava extract is superior to placebo as a symptomatic treatment for anxiety. Therefore, kava extract is an herbal treatment option for anxiety that is worthy of consideration.

A review of studies suggests that kava may provide short-term relief from anxiety symptoms.

Aim of the study: To determine if Kava Kava is an effective treatment for combating symptoms of anxiety despite warnings of hepatotoxicity from the Centers for Disease Control and Prevention (CDC).

Results: Kava Kava was shown to be more effective than placebo in 3 of the 7 trials. A final risk ratio of 1.50 (95% CI: 1.12, 2.01) from responder rates was calculated in favor of the intervention from 5 clinical trials (n = 330). Adverse events were shown to be the same as placebo (P = 0.574), and laboratory values analyzing hepatotoxicity were no different when compared to baseline except in two studies. Kava Kava appears to be a short-term treatment for anxiety, but not a replacement for prolonged anti-anxiety use. Although not witnessed in this review, liver toxicity is especially possible if taken longer than 8 weeks.

The review found high-quality evidence supporting the use of kava for anxiety disorders.

Aim of the study: This paper provides a 10-year update of the 2007 systematic review of herbal medicines studied in a broad range of psychiatric disorders, including depression, anxiety, obsessive-compulsive, seasonal affective, bipolar, psychotic, phobic, somatoform, and attention-deficit hyperactivity disorders.

Results: Other encouraging herbal medicines with preliminary evidence include Curcuma longa (turmeric) in depression, Withania somnifera (ashwagandha) in affective disorders, and Ginkgo biloba (ginkgo) as an adjunctive treatment in Schizophrenia. Although depression and anxiety are commonly researched, many other mental disorders still require further prospective investigation. Although the previous review suggested increasing the adjunctive study of select herbal medicines with pharmaceuticals, this was still only found to sparingly occur in research designs. Aside from this, future focus should involve the incorporation of more biomarker analysis, in particular pharmacogenomics, to determine genetic factors moderating response to herbal medicines.

A review found that kava may help with generalized anxiety disorder.

Aim of the study: Mood and anxiety disorders pose significant health burdens on the community. Kava and St. John's wort (SJW) are the most commonly used herbal medicines in the treatment of anxiety and depressive disorders, respectively.

Results: Current evidence supports the use of SJW in treating mild-moderate depression, and for kava in treatment of generalized anxiety. In respect to the other disorders, only weak preliminary evidence exists for use of SJW in SAD. Currently there is no published human trial on use of kava in affective disorders, or in OCD, PTSD, PD, or SP. These disorders constitute potential applications that warrant exploration. Current evidence for herbal medicines in the treatment of depression and anxiety only supports the use of Hypericum perforatum for depression, and Piper methysticum for generalized anxiety.

An observational study recorded improvements in nervous tension and restlessness after short-term use of kava.

Aim of the study: Prior to the kava ban of 2002, the indication for kava (Piper methysticum) extracts defined by the German Commission E was "nervous anxiety, tension and restlessness". In 2000, an observational trial was started in Germany with the aim of defining symptoms of these indications best treated with kava extract. The trial was interrupted and archived "unevaluated" in 2001 due to the upcoming safety debate on kava.

Results: The median duration of treatment was 28 days. All individual symptoms showed significant and clinically relevant improvements. The most effective results were seen for nervous tension and restlessness, with better effects in patients with acute versus chronic complaints. The safety of the treatment was found to be excellent, which included the assessment of laboratory data. Overall, the study confirms the effective and safe short-term use of kava in the Commission E-defined indication of "nervous anxiety, tension and restlessness", especially in other than chronic cases. The clinical use of kava might be translated into context-related phobias according to ICD-10 F40, or to nervous tension (ICD10 R45.0) or restlessness and excitation (ICD-10 R45.1).

The review indicates that randomized trials support the effectiveness of kava for generalized anxiety disorder.

Aim of the study: Generalized anxiety disorder (GAD) often begins during adolescence or early adulthood and persists throughout the lifespan. Randomized controlled trials support the efficacy of selective serotonin and selective serotonin norepinephrine reuptake inhibitors (SSRIs and SNRIs, respectively), as well as benzodiazepines, azapirones, anti-adrenergic medications, melatonin analogs, second-generation antipsychotics, kava, and lavender oil in GAD.

Results: Areas covered: The authors review the literature concerning pharmacotherapy for pediatric and adult patients with GAD with specific commentary on the efficacy and tolerability of selected agents in these age groups. The authors describe an algorithmic approach to the pediatric and adult patient with GAD and highlight considerations for the use of selected medications in these patients. Expert opinion: In adults with GAD, SSRIs and SNRIs represent the first-line psychopharmacologic treatment while second-line pharmacotherapies include buspirone, benzodiazepines, SGAs, and pregabalin. In pediatric patients with GAD, SSRIs should be considered the first line pharmacotherapy and psychotherapy enhances antidepressant response.

A review describes reduced anxiety after taking kava-kava, without a decline in mental performance.

Aim of the study: Kava-kava (Piper methysticum G. Forster) has been used in social and ceremonial life in the Pacific islands from ancient times for the soporific and narcotic effects.

Results: Although kava-kava has been found to be very effective, well tolerated, and non-addictive at therapeutic dosages, potential side effects can occur when very high doses are taken for extended periods. In addition, in the last two years unexpected high liver toxicity has been reported in two patients. Until now no studies support the liver toxicity of kavalactones and it is unknown which compound could have provoked the liver disease. On the other hand, it should be possible that unknown or unexpected constituents are the responsible or contributed to the liver toxicity.

The review found that, in clinical studies, kava was among the plants with the most positive results for relieving anxiety.

Aim of the study: Exploration of new drugs targeting anxiety treatment is a major concern worldwide. Medicinal plants are being used as a potential source of novel drugs for anxiety disorders.

Results: Valeriana officinalis, Citrus aurantium, Commelina benghalensis, Achyranthes aspera, Mimosa pudica, Achillea millefolium, Nymphaea alba, Leonurus cardiac, Camellia sinensis, Turnera aphrodisiaca, Crataegus oxyacantha and Piper methysticum showed promising effects on anxiety in animal models. In clinical studies, passion flower, kava, valerian, St John's wort, and hwagandha showed the most positive results. More studies are needed for the exploration of the antianxiety of medicinal plants. In drugs derived from natural sources have explored many components that are playing an essential role in curing anxiety disorders and associated complications.
