Folic acid in pregnancy — are all forms equally good?

Sources: 6 · Article: 8 523 characters

Andrzej Frydrychowski recommends L-methylfolate, which is meant to bypass the problem related to methylation.

  • According to him, a genetic error makes methylation difficult in about 40% of people, causing folic acid to be absorbed improperly.
  • Frydrychowski says folic acid is needed during pregnancy and affects homocysteine levels and the risk of neural tube defects in the fetus.
  • Henryk Różański explains that the body converts folic acid into its active form — folinic acid.

Andrzej Frydrychowski

L-methylfolate is meant to bypass the problem caused by a genetic error that makes methylation difficult.

„Some of us (about 40%) have a genetic error that means methylation does not occur and folic acid is absorbed improperly. Synthetic folic acid and natural folates are not active metabolic forms. It is therefore worth choosing L-methylfolate, which allows us to bypass this defect.”Niektórzy z nas (około 40%) mają błąd genetyczny, który powoduje, że metylacja nie zachodzi i kwas foliowy niewłaściwie się wchłania. Syntetyczny kwas foliowy i naturalne foliany nie są bowiem aktywnymi formami metobolicznymi. Warto więc wybierać L-metylowany kwas foliowy, dzięki czemu omijamy tę wadę.

A genetic error in about 40% of people is said to prevent methylation and cause folic acid to be absorbed improperly.

Folic acid is needed during pregnancy and affects homocysteine levels and the risk of neural tube defects in the fetus.

„I would also like to draw attention to vitamin B9, or folic acid, which is essential for pregnant women. It affects homocysteine levels and prevents neural tube defects in the fetus. It can be found in dark green leafy vegetables (spinach, kale, lettuce), broccoli, asparagus, avocado, citrus fruits, whole-grain cereals, and yeast.”Chciałbym zwrócić uwagę także na witaminę B9, czyli kwas foliowy, który jest niezbędny u kobiet w ciąży. Wpływa na poziom homocysteiny i zapobiega wadom cewy nerwowej płodu. Możemy ją znaleźć w: ciemnozielonych warzywach liściastych (szpinak, jarmuż, sałata), brokułach, szparagach, awokado, owocach cytrusowych, zbożach z pełnego przemiału i w drożdżach.
Henryk Różański

The body converts folic acid into its active form, folinic acid.

„Folic acid, or pteroylglutamic acid, is converted in the human body into its active form — folinic acid. Folinic acid is a coenzyme essential for the synthesis of nucleic acids and proteins. It is necessary for cell division, growth, tissue development, and regeneration.”Kwas foliowy, czyli pteroiloglutaminowy przekształcany jest w organizmie człowieka do czynnej formy – kwasu folinowego. Kwas folinowy to koenzym niezbędny do syntezy kwasów nukleinowych i białek. Warunkuje podział komórek, proces wzrostu, rozwoju tkanek oraz procesy regeneracji.

A meta-analysis found that replacing folic acid with 5-MTHF reduced the level of unmetabolized folic acid in the blood, but the effects on other measures were unclear.

Aim of the study: Background: Mandatory folic acid (FA) fortification prevents neural tube defects but leads to unmetabolized folic acid (UMFA) in the blood, because the liver enzyme dihydrofolate reductase (DHFR) reduces FA only slowly. Whether UMFA itself, separate from high total folate, has biological effects remains unclear.

Results: For red blood cell folate, the two available trials showed conflicting directions of effect but were insufficient to establish equivalence or non-inferiority. For tHcy, no difference between forms was detected. With only two to three trials per outcome, no prespecified margins, and no trial designed for this purpose, the evidence cannot support a claim of equivalence or non-inferiority for either outcome, and clinical benefit remains unproven. Current evidence does not establish that UMFA independently causes harm.

The review suggests that reduced forms of folate may offer benefits in selected groups, including pregnancies with positive FRAA antibody results, but larger studies are needed.

Aim of the study: Folate (vitamin B9) and vitamin B12 are essential micronutrients for cell growth, DNA synthesis, methylation, and one-carbon metabolism, and are especially important for early development. This review begins with an overview of vitamins essential to development, with a focus on folate and B12, followed by a detailed examination of folate sources, chemical forms, absorption, bioavailability, and genetic influences on folate metabolism and transport.

Results: We then discuss neurodevelopmental disorders, focusing on autism spectrum disorder (ASD), and summarize evidence on folate's role in brain formation during pregnancy, maternal folate deficiency as a contributor to autism risk, and the critical periods when folate status may have the greatest influence on neurodevelopmental outcomes. We focus specifically on clinical trial evidence: the detection of folate deficiency or insufficiency in children with ASD and treatment with leucovorin (folinic acid), as well as the detection of folate deficiency risk in pregnancy, particularly through screening for folate receptor alpha autoantibodies (FRAA), and treatment with calcium folinate. Recent randomized, placebo-controlled trials suggest that reduced forms of folate may offer benefits over folic acid in selected groups, including children with ASD and FRAA-positive pregnancies, though larger confirmatory studies are needed.

The review points to some advantages of 5-MTHF and folinic acid, but folic acid is the only form with proven effectiveness in large studies on preventing neural tube defects.

Aim of the study: Folate compounds are crucial for DNA and RNA synthesis, homocysteine regulation, and epigenetic methylation. However, significant differences exist between 5-methyltetrahydrofolate (5-MTHF), folinic acid (CHO-THF), and synthetic folic acid (sFA).

Results: CHO-THF shows promise in managing autism spectrum disorders by affecting neurotransmission and neurometabolic pathways. Despite these advantages, sFA remains the only folate form with proven effectiveness in large randomized clinical trials for preventing neural tube defects (NTDs), and it continues to play a key role in public health strategies. Use of sFA at doses above 1000 µg/day requires monitoring to avoid masking B12 deficiency. For personalized care or high-risk cases, 5-MTHF and CHO-THF should be the preferred options, ideally combined with vitamin B12 supplementation.

„Folate (with B12) — Neural tube closure is a one-time event. Folic acid taken around conception prevents neural tube defects (NTDs); this is now the standard of care worldwide because it is impossible to catch up later. Low maternal B12 independently increases NTD risk and is associated with poorer early neurodevelopment; adequate levels of both folate and B12 are safest (19-23).”Folate (with B12) - Neural Tube Closure is a One-Time Event Peri-conceptional folic acid prevents neural tube defects (NTDs); this is now standard of care worldwide because catch-up later is impossible. Low maternal B12 independently raises NTD risk and is associated with poorer early neurodevelopment; folate and B12 adequacy together are safest (19-23).
