The keto diet—a revolution in weight loss or a dangerous fad?

Sources: 13 · Article: 17 350 characters

The Rodzeń brothers report that low-carbohydrate diets are usually associated with greater weight loss than other diets, even over a period of two years.

  • In a one-year study, people on a low-carbohydrate diet lost an average of 3.5 kg more than people on a low-fat diet.
  • According to the Rodzeń brothers, ketosis suppresses appetite, and the keto diet limits carbohydrates, often to less than 20 g per day.
  • M. Williamson, N. Moustaid-Moussa, and L. Gollahon describe possible benefits of keto, while Paul Clayton discusses its effects on gut flora and inflammation.

Meta-analyses indicate that low-carbohydrate diets usually lead to greater weight loss than other diets, even over a period of two years.

„A series of meta-analyses of randomized controlled trials (RCTs), considered the strongest and most reliable evidence, reached the same conclusion: low-carbohydrate diets usually outperform other diets for weight loss over periods of up to two years.”Szereg metaanaliz przeprowadzonych w randomizowanych badaniach kontrolowanych (RCT), uważanych za najsilniejsze i najbardziej wiarygodne dowody, doprowadziło do tego samego wniosku: diety niskowęglowodanowe zwykle przewyższają inne diety w zakresie utraty wagi przez okres do dwóch lat.

The Rodzeń brothers say that ketosis suppresses appetite and present this lifestyle as an effective way to lose weight healthily.

„These are the facts: studies have shown that being in ketosis suppresses appetite. Why? Because by reducing the amount of carbohydrates you eat, you suppress your appetite, lower your insulin levels, and become a fat-burning machine. So it should come as no surprise that this is the most effective lifestyle for healthy weight loss.”Takie są fakty, badania wykazały, że bycie w ketozie hamuje apetyt. Dlaczego?

The Rodzeń brothers describe keto as a diet that limits carbohydrates, often to less than 20 g per day, and leads to fat being burned for energy.

„The main principle of this lifestyle is to limit carbohydrates to below 50 g/day, often below 20 g/day. By gradually reducing the amount of carbohydrates you eat, you increase your intake of high-quality protein and fat (until you feel full). This puts your body into a metabolic state called ketosis, in which fat from food and from the body is burned for energy.”Główną zasadą tego stylu życia jest ograniczenie węglowodanów do poniżej 50 g/dobę, często poniżej 20 g/dobę. Obniżając stopniowo ilość spożywanych węglowodanów, zwiększasz ilość wysokiej jakości białka i tłuszczu (do sytości). W wyniku takiego postępowania, wprowadzasz organizm w stan metaboliczny zwany ketozą, w którym tłuszcz pochodzący z diety i ciała jest spalany w celu uzyskania energii.
Bracia Rodzeń

The Rodzeń brothers call the belief that the keto diet is dangerous and harmful one of the myths.

„Even at university, we are taught outdated information and told to give our patients ineffective advice: Avoid animal fat, eat frequently, the keto diet is dangerous and harmful, you have to eat carbohydrates because they are essential for life, and your body is a calorie-burning machine.”Już na studiach uczą nas przestarzałej wiedzy i każą naszym pacjentom dawać nieskuteczne zalecenia: Unikaj tłuszczu zwierzęcego, jedz często, dieta keto jest niebezpieczna i szkodliwa, trzeba jeść węglowodany bo są niezbędne do życia a Twoje ciało to maszyna do spalania kalorii.
Bracia Rodzeń

The Rodzeń brothers use the slogan “The Keto Diet Kills” in a series they present as debunking myths about weight loss and nutrition.

„Breakfast is the most important meal of the day!!!! The Keto Diet Kills. Eat 6 times a day and believe all the experts. In the series “Breaking Out of the Matrix,” we debunk myths from the weight-loss and nutrition industry and open your eyes to the truth. The difficult truth and the complicated world of scientific research and dietary recommendations.”Śniadanie to najważniejszy posiłek dnia !!!! Dieta Keto zabija. M. Williamson i N. Moustaid-Moussa i L. Gollahon

M. Williamson, N. Moustaid-Moussa, and L. Gollahon indicate that the ketogenic diet may limit visceral fat accumulation and some problems associated with diabetes and hypertension.

„Reduction of glucose, energy/calorie restriction, and ketogenic diet can be beneficial in reducing visceral fat accumulation, ROS generation, lipid peroxidation, symptoms of diabetes, hypertension, and even COVID-19 complications. It has been suggested that reducing glucose and carbohydrates by ketogenic diet can reduce the amount of energy produced from aerobic glycolysis and therefore switch metabolism to be more advantageous to non-inflammatory phenotype or M2 macrophages ().”Reduction of glucose, energy/calorie restriction, and ketogenic diet can be beneficial in reducing visceral fat accumulation, ROS generation, lipid peroxidation, symptoms of diabetes, hypertension, and even COVID-19 complications. Paul Clayton

Paul Clayton describes how the ketogenic diet changes gut flora, reduces the number of some butyrate-producing bacteria, and at the same time limits some cells and substances associated with inflammation.

„The ketogenic diet has a significant effect on the microbiota (33) and dramatically reduces saccharolytic, butyrate-producing bacterial species such as bifidobacteria (33, 34). But there is another aspect to the matter. During ketosis, the liver produces ketone bodies: acetoacetate, acetone, and butyrate. In addition, the ketogenic diet reduces the population of pro-inflammatory intestinal Th17 cells (33), pro-inflammatory group 3 innate lymphoid cells (35), and the production of pro-inflammatory mediators (33), and therefore also inflammation in the nervous system (36).”Dieta ketogeniczna ma znaczący wpływ na mikroflorę (33) i dramatycznie redukuje gatunki sacharolityczne bakterii wytwarzające maślan, takie jak bifidobakterie (33, 34).Ale jest też inny aspekt sprawy. Podczas ketozy wątroba wytwarza ciała ketonowe: acetooctan, aceton i maślan. Poza tym dieta ketogeniczna zmniejsza: populację jelitowych prozapalnych komórek Th17 (33), prozapalnych wrodzonych komórek limfoidalnych grupy 3 (35), produkcję mediatorów prozapalnych (33), a zatem i stan zapalny układu nerwowego (36).

A meta-analysis found that the ketogenic diet helped overweight people with type 2 diabetes reduce their body weight and improved some metabolic markers.

Aim of the study: A ketogenic diet, characterized by low calories with high levels of fat, adequate levels of protein, and low levels of carbohydrates, has beneficial effects on body weight control in overweight patients. In the present study, a meta-analysis was conducted to investigate the role of a ketogenic diet in body weight control and glycemic management in overweight patients with type 2 diabetes mellitus (T2DM).

Results: In summary, we systematically reviewed articles from the Embase, PubMed, Web of Science and Cochrane Library databases and obtained eight randomized controlled trials for meta-analysis. The results show that a ketogenic diet had significantly beneficial effects on the loss of body weight (SMD, -5.63, p = 0.008), the reduction of waist circumference (SMD, -2.32, p = 0.04), lowering glycated hemoglobin (SMD, -0.38, p = 0.0008) and triglycerides (SMD, -0.36, p = 0.0001), and increasing high-density lipoproteins (SMD, 0.28, p = 0.003). Overall, these results suggest that a ketogenic diet may be an effective dietary intervention for body weight and glycemic control, as well as improved lipid profiles in overweight patients with T2DM. Hence, a ketogenic diet can be recommended for the therapeutic intervention of overweight patients with T2DM.

A review with a meta-analysis found that a very-low-calorie ketogenic diet promoted weight loss and improved body composition and some metabolic markers.

Aim of the study: The very low-calorie ketogenic diet (VLCKD) has been recently proposed as an appealing nutritional strategy for obesity management. The VLCKD is characterized by a low carbohydrate content (<50 g/day), 1-1.5 g of protein/kg of ideal body weight, 15-30 g of fat/day, and a daily intake of about 500-800 calories.

Results: Of the 645 articles retrieved, 15 studies met the inclusion criteria and were reviewed, revealing 4 main findings. First, the VLCKD was shown to result in a significant weight loss in the short, intermediate, and long terms and improvement in body composition parameters as well as glycemic and lipid profiles. Second, when compared with other weight loss interventions of the same duration, the VLCKD showed a major effect on reduction of body weight, fat mass, waist circumference, total cholesterol and triglyceridemia as well as improved insulin resistance. Third, although the VLCKD also resulted in a significant reduction of glycemia, HbA1c, and LDL cholesterol, these changes were similar to those obtained with other weight loss interventions. Finally, the VLCKD can be considered a safe nutritional approach under a health professional's supervision since the most common side effects are usually clinically mild and easily to manage and recovery is often spontaneous.

A meta-analysis found that, with similar prescribed energy intake, the ketogenic diet was associated with slightly greater weight loss than a higher-carbohydrate diet.

Aim of the study: Background/Objectives: Whether ketogenic diets provide greater weight-loss benefits than higher-carbohydrate diets when prescribed energy intake or targeted energy deficit are comparable remains uncertain. This systematic review and meta-analysis evaluated their effects on body weight change in adults with overweight or obesity.

Results: In three trials meeting the prescribed protein-matching criterion, the effect remained directionally consistent but was imprecise and compatible with no difference. Certainty of evidence was low because of risk of bias and imprecision. Conclusions: Under comparable prescribed energy conditions, ketogenic diets may produce a small additional reduction in body weight. However, predominantly short-term evidence, uncertainty regarding achieved energy and protein intakes, the absence of a synthesis of body composition, and low certainty of evidence limit confidence in the magnitude, durability, and clinical significance of this advantage.

In a three-month study, people on a ketogenic diet lost more weight than those following a calorie-restricted Mediterranean diet.

Aim of the study: Studies evaluating the effects of novel, alternative dietary approaches for weight loss compared with the Mediterranean diet (MedDiet) are lacking. We aimed to evaluate the effects of diets with varying ketogenic potential, i.e., a very-low carbohydrate diet (ketogenic diet, KD), time-restricted eating (TRE), and modified alternate-day fasting (mADF) on weight loss in obesity, compared with a MedDiet.

Results: The mean age was 45.7 years (SD 10.7), and 70.6% were women. One hundred forty participants completed the study. Significant differences in weight loss from baseline to 3 months were found between KD and the control group [- 3.78 kg (- 5.65 to - 1.91 kg)], between mADF and the control group [- 3.14 kg (- 4.98 to - 1.30 kg)], and between lTRE and the control group [- 2.27 kg (- 4.13 to - 0.40 kg)], but not between eTRE and the control group [- 1.22 kg (- 3.07 to 0.64 kg)]. These results suggest that a calorie-restricted KD, mADF, or lTRE may be more effective for weight loss than a calorie-restricted MedDiet in obesity. Further research is needed to evaluate the long-term feasibility and efficacy of these dietary interventions compared with the MedDiet.

In a six-month study, a healthy ketogenic diet resulted in greater average weight loss than an energy-restricted diet.

Aim of the study: Introduction: The ketogenic diet (KD) is widely used for weight management by reducing appetite, enhancing fat oxidation, and facilitating weight loss. However, the high content of total and saturated fats in a conventional KD may elevate low-density lipoprotein (LDL)-cholesterol levels, a known risk factor for cardiovascular diseases, highlighting the need for healthier alternatives.

Results: 0.6 ± 11.5 IU/L, p = 0.01)], with no increase in LDL-cholesterol (-0.12 ± 0.60 mmol/L vs. -0.04 ± 0.56 mmol/L, p = 0.97) observed in either group. Conclusions: The HKD was more effective than the ERD in promoting weight loss and improving cardiometabolic outcomes without elevation in LDL-cholesterol. It can be recommended for therapeutic intervention in patients with obesity.
