Back pain — is surgery really the only option?

Sources: 17 · Article: 20 898 characters

Jan Jaśkowski recommends starting with conservative treatment: a gradual return to movement and rehabilitation, and considering surgery only in specific cases.

  • Among the proposed methods, Jan Jaśkowski mentions combining manual therapy with movement, along with exercise and changes to everyday habits.
  • Staying in bed for a long time does not help, massage may have a short-lived effect, and exercises found online may make the condition worse.
  • If a patient has urinary or bowel incontinence, or weakness in a limb, Jan Jaśkowski recommends urgent surgery.

What the authors write

Jan Jaśkowski

Conservative treatment, including physical activity, should be tried before surgery; surgery is rarely necessary.

„Unfortunately, patients are referred for spinal surgery too hastily, for example for spinal fusion, which is also very expensive. Scientific studies show that the medium- and long-term results of surgery are no better than those of conservative treatment.

A long period of bed rest may be associated with more pain, greater limitations, and longer absences from work.

„Although rest may initially bring relief, staying in bed for a prolonged period is not helpful. It is associated with more pain, greater limitations, and longer absences from work. Surgery is rarely necessary to treat patients with back pain.”Mimo że początkowo odpoczynek może przynosić ulgę w cierpieniu, wydłużone leżenie nie jest pomocne, jest związane z większym bólem, większymi ograniczeniami i dłuższą absencją z pracy. Operacja rzadko jest niezbędna do wyleczenia pacjentów z bólu kręgosłupa.
Jan Jaśkowski

Exercises are intended to stimulate receptors, and Jan Jaśkowski recommends doing them mainly while standing or sitting.

„Remember that these exercises should mainly be done in an upright position—standing or sitting. Our muscles and joints work differently when we are upright than when we are lying down. The load on the spine is different, and we maintain our balance differently.”Pamietajmy, że ćwiczenia te powinny być wykonywane, głównie w pozycji pionowej — stojącej lub siedzącej. Nasze mięśnie i stawy inaczej funkcjonują w pionie, a inaczej, kiedy leżymy. Inne jest obciążenie kręgosłupa i inaczej utrzymujemy wówczas równowagę.
Jan Jaśkowski

Jan Jaśkowski recommends combining manual therapy with movement therapy and identifies rehabilitation as the first step, except in specific cases.

„A good osteopath or physiotherapist can help even in very difficult cases. It is worth looking for a specialist who combines manual therapy with movement therapy. According to the National Institute for Clinical Excellence mentioned earlier, this combination of therapies produces the best results.”Dobry osteopata czy fizjoterapeuta potrafi pomóc w nawet bardzo trudnych przypadkach. Warto poszukać specjalisty łączącego terapię manualną z terapią ruchem. Według wspomnianego już National Institute for Clinical Excelence takie skojarzenie terapii daje najlepsze efekty.
Jan Jaśkowski

If a patient has urinary or bowel incontinence, or weakness in a limb, Jan Jaśkowski recommends immediate surgery.

„In cases of uncontrolled urination or bowel movements, or weakness in a limb, surgery should be performed without delay. In all other cases, however, treatment should always start with rehabilitation. A good osteopath or physiotherapist can help even in very difficult cases.”W przypadku niekontrolowanego oddawania moczu i kału lub niedowładu kończyny należy bezwłocznie poddać się operacji.

Jan Jaśkowski advises against choosing exercises from the internet on your own and recommends basing treatment on reliable knowledge and proven methods.

„This applies even more to people without the right diagnosis. It is never possible to know with 100% certainty how a patient's body will respond to a particular treatment. That is why treatment should be based on reliable medical knowledge and methods tested in scientific studies, not on stories from neighbors or friends.”A tym bardziej laikom bez odpowiedniej diagnozy.

Jan Jaśkowski describes osteopathic treatment as addressing mechanical problems, exercise, posture, and other aspects of a patient's life.

„Osteopaths consider back pain in the context of a patient's life. Treatment aims to improve many factors that affect health, such as exercise, posture, the workplace, diet, and more. This comprehensive approach aims not only to reduce current pain, but also to prevent it from returning and to improve the body's capacity and potential.”Osteopaci patrzą na bóle kręgosłupa w kontekście życia pacjenta, a leczenie ma na celu usprawnienie wielu apsektów wpływających na zdrowie, takich jak ćwiczenia, postawa, stanowisko pracy, dieta i inne.

Massage can relax muscles and increase joint mobility, but its effects may be short-lived.

„However, these treatments are not always effective enough. Massage helps relax tense muscles and increase joint mobility, but its effects may be short-lived. This is because massage does not remove the cause of muscle tension or restricted movement.”Jednak ich skuteczność nie zawsze jest zadowalająca.

Jan Jaśkowski notes that imbalances in the hip muscles and weak gluteal muscles may be linked to back pain and can be corrected.

„This is extremely important because it means that the original cause of back pain may lie in other joints. From an osteopathic treatment perspective, these differences are easy to identify and, if there are no significant factors that predispose someone to such imbalances, they are relatively easy to correct.”Jest to niezwykle ważne ponieważ oznacza, że pierwotna przyczyna bólu kręgosłupa może leżeć w innych stawach.

Jan Jaśkowski describes gentle manipulation of the internal organs as a way to reduce adhesions and increase spinal mobility.

„Adhesions in the abdominal cavity can be reduced through gentle manipulation of the internal organs. In this way, osteopaths increase spinal mobility and reduce the likelihood of overloading and injury. Doctors successfully treat many patients suffering from back pain.”Zrosty w jamie brzusznej można zredukować poprzez delikatne manipulacje na narządach wewnętrznych. W ten sposób osteopaci zwiększają ruchomość kręgosłupa, zmniejszając możliwość występowania przeciążeń i urazów. Lekarze z powodzeniem leczą wielu pacjentów cierpiących na bóle kręgosłupa.
Jan Jaśkowski

Jan Jaśkowski recommends taking frequent breaks from sitting and gradually preparing the spine for lifting and carrying heavy objects.

„Just as we may feel knee pain after a long walk, we may also feel back pain after lifting heavy or awkward objects. Just as there are different styles of running, there are different ways of lifting, depending on the person. The key is to gradually train the spine to lift and carry objects of different weights and shapes.”Tak samo jak możemy odczuwać ból kolana po długim marszu, tak samo możemy odczuwać ból kręgosłupa po dźwiganiu ciężkich lub nieporęcznych przedmiotów.

Jan Jaśkowski notes that conservative treatment can be just as effective as spinal stabilization surgery.

„However, numerous studies comparing surgical spinal stabilization with conservative treatment show that the two approaches are currently equally effective. As early as 1993, scientific studies found that this surgery was effective in only about 30% of cases.”Jednak liczne badania porównujące leczenie poprzez chirurgiczną stabilizację kręgosłupa z leczeniem zachowawczym dowodzą, że skuteczność obu metod obecnie jest taka sama.

Jan Jaśkowski says that surgery can involve complications and is just as effective as conservative treatment.

„It should be added that rehabilitation options are much more limited after this surgery. Surgical treatment carries a risk of complications. On average, 25% of surgical procedures end in complications, ranging from infection of the surgical wound and deep vein thrombosis to damage to the spinal dura and leakage of cerebrospinal fluid. Surgical treatment is just as effective as conservative treatment.

A review found that nerve mobilization may reduce pain and improve function and disability in the short term.

Aim of the study: Low back pain can present with radicular pain caused by lumbosacral nerve root pathology. Neural mobilization (NM) is a treatment technique used to treat low back and radicular pain (LBRP). To evaluate the effectiveness of NM interventions in improving pain, disability, and function in adults with LBRP.

Results: NM may be an effective tool for short-term improvements in pain, function, and disability associated with LBRP. Additional high quality research is needed.

Stabilizing exercises reduced pain and functional limitations, and the improvement was maintained during the 30-month follow-up.

Aim of the study: To determine the efficacy of a specific exercise intervention in the treatment of patients with chronic low back pain and a radiologic diagnosis of spondylolysis or spondylolisthesis. A recent focus in the physiotherapy management of patients with back pain has been the specific training of muscles surrounding the spine (deep abdominal muscles and lumbar multifidus), considered to provide dynamic stability and fine control to the lumbar spine.

Results: After intervention, the specific exercise group showed a statistically significant reduction in pain intensity and functional disability levels, which was maintained at a 30-month follow-up. The control group showed no significant change in these parameters after intervention or at follow-up.

Physiotherapy improved pain and function and reduced the size of the herniation; additional decompression showed no advantage over other physiotherapy methods.

Aim of the study: The aim of the study determining whether or not Non-invasive Spinal Decompression Therapy (NSDT) was effective in resorption of herniation, increasing disc height in patients with lumbar disc herniation (LHNP).

Results: Both treatments had positive effect for improving pain, functional restoration and reduction in thickness of herniation. Although reduction of herniation size was higher in SG than CG, no significant differences were found between groups and any superiority to each other (p> 0.05). This study showed that patients with LHNP received physiotherapy had improvement based on clinical and radiologic evidence. NSDT can be used as assistive agent for other physiotherapy methods in treatment of lumbar disc herniation.

In the case report, pain after an injury resolved following conservative treatment involving compression.

Aim of the study: Morel-Lavallée lesions (MLLs) are closed traumatic degloving injuries caused by separation of the subcutaneous tissue from the underlying fascia. Although most commonly described around the greater trochanter and pelvis, lumbar MLLs are rare and may be overlooked in patients presenting with post-traumatic low back pain.

Results: Interval ultrasound demonstrated only a minimal residual collection without drainable fluid, and aspiration was deferred. His symptoms progressively improved and completely resolved within four weeks. This case highlights lumbar MLL as an uncommon cause of severe post-traumatic low back pain despite reassuring osseous imaging. Early recognition and appropriate soft-tissue imaging can guide management, help avoid unnecessary invasive treatment, and allow selected small acute lesions to resolve successfully with conservative care.

A meta-analysis found that conservative treatments for back and pelvic pain during pregnancy reduced pain and functional limitations.

Aim of the study: Pregnancy-associated low back and pelvic pain is a common musculoskeletal condition associated with substantial pain, impaired mobility, and functional limitation during pregnancy. Pharmacologic pain management is constrained by maternal and fetal safety considerations, increasing reliance on conservative musculoskeletal interventions.

Results: Six randomized controlled trials met inclusion criteria for qualitative synthesis, of which four studies involving 254 participants contributed directly extractable data for the primary quantitative meta-analyses. Conservative musculoskeletal interventions were associated with significant reductions in pain intensity (SMD -0.85, 95% CI -1.18 to -0.53; P < 0.001; I² = 34%) and functional disability (SMD -0.84, 95% CI -1.31 to -0.37; P < 0.001; I² = 66%). Subgroup analysis demonstrated numerically larger pain effects for device-assisted interventions, followed by acupuncture-based and manual or manipulative approaches, although subgroup differences were not statistically significant (P = 0.26). One trial was judged at high risk of bias, while the remaining studies were classified as having some concerns. Conservative musculoskeletal interventions were associated with improvements in pain intensity and functional disability among women with pregnancy-associated low back and pelvic pain.

In people with vertebral compression fractures, both conservative treatment and vertebroplasty reduced pain and improved quality of life.

Aim of the study: BACKGROUND The optimal treatment strategy for osteoporotic vertebral compression fractures (OVCFs) remains unclear. This single-center study aimed to compare outcomes of percutaneous vertebroplasty (PVP) and conservative management in 55 patients with OVCF.

Results: No significant difference in complications was observed between groups (P>0.05). CONCLUSIONS Both PVP and conservative treatment relieved pain and improved quality of life in patients with OVCF. PVP provided more rapid pain relief and functional recovery. Although conservative treatment resulted in slower improvement and longer duration of bed rest, long-term clinical outcomes and complication rates were similar between groups.

The case report indicates that conservative treatment with shock-wave therapy and acupuncture was associated with almost complete resolution of radiating pain and numbness.

Aim of the study: While conservative management often improves lumbar disc herniation (LDH) symptoms and promotes spontaneous resorption over time, the clinical value of extracorporeal shock wave therapy (ESWT) in accelerating this process remains underexplored.

In two reported cases of refractory sacroiliac joint pain, substantial pain relief persisted after a PRP injection.

Aim of the study: Sacroiliac joint (SIJ) pain is an important source of chronic axial low back and buttock pain. Initial treatment commonly includes activity modification, physical therapy, nonsteroidal anti-inflammatory drugs (NSAIDs), and other nonopioid medications. Interventional treatment options may include corticosteroid injections, radiofrequency ablation, and, in selected refractory cases, SIJ fusion.

Results: PRP injection was associated with sustained improvement in pain in two patients with refractory SIJ pain after failure of multiple conventional treatments. These cases provide additional real-world clinical observations supporting further investigation of PRP as a potential nonsurgical option for carefully selected patients with persistent SIJ pain.

„Fortunately for Kennedy, Max had acquired a great deal of expertise treating back pain. In fact, as a medical intern in Berlin in the 1920s, Max was assistant to Berlin's leading surgeon, Dr. Auguste Bier, who performed the very first spinal anesthetic in 1898. In addition to being a research chemist in experimental medicine, Max was also a surgeon as well as a general practitioner. Dr. Jacobson first demonstrated an exercise to strengthen the back, and then treated Kennedy with a combination of an anti-inflammatory, a local anesthetic, and vitamin B-12, injected in an exact spot on either side of the spine.”Fortunately for Kennedy, Max had acquired a great deal of expertise treating back pain. „Examples of Other DMSO Uses I did use DMSO for several months for a lower "stiff back" pain. I felt it was the only treatment that made a difference. Eventually, I got tired of waiting for it to soak in and quit the procedure. The back pain has been substantially less since. The solid form of DMSO is Methylsulfonylmethane (MSM), an over-the-counter supplement. MSM [(CH3)2SO2] is DMSO plus one additional oxygen. Now I supplement with MSM most mornings, on the speculation it is a convenient maintenance dose that helps keep the back pain minimized, so far, so good.
