The liver — do milk thistle and herbs really cleanse it?

Sources: 15 · Article: 13 987 characters

Henryk Różański recommends an herbal mixture to support liver detoxification, while Hubert Czerniak mentions milk thistle for cleansing the body, not the liver itself.

  • Henryk Różański warns that adding turmeric to food does not justify the claim that it will protect liver tissue.

Silymarin from milk thistle is said to stabilize liver cell membranes, protect the liver from toxins, and support regeneration.

„Milk thistle owes its medicinal properties to silymarin. [?] Silymarin stabilizes the cell membranes of liver hepatocytes, thereby protecting the liver from toxic compounds and stimulating its ability to regenerate [2]; as with all diseases, diet plays an important role in treatment.”Ostropest plamisty swoje właściwości lecznicze zawdzięcza sylimarynie. [?]

Combining silymarin with artichoke is said to strengthen the protective effect on liver tissue.

„I consider this an excellent solution, because the active compounds in both complexes complement each other and enhance their hepatoprotective (protecting liver tissue), bile-stimulating, bile-producing, anti-inflammatory, and lipotropic properties. In higher doses, the preparation has a laxative effect.”Uważam to za doskonałe rozwiązanie, bowiem oba kompleksy związków aktywnych wzajemnie się uzupełniają i wzmagają we właściwościach hepatoprotekcyjnych (ochraniających miąższ wątroby), żółciopędnych, żółciotwórczych, przeciwzapalnych i lipotropowych. W większych dawkach preparat działa przeczyszczająco.
Henryk Różański

Veronica has been credited with a protective effect on the liver and use in cases of liver damage, inflammation, cirrhosis, and fatty liver.

„Medicinal effects. Due to the presence of iridoids and phenolic acids in the raw material: antispasmodic; protective of liver tissue (including against mycotoxins)—hepatoprotective; protective of nerve cells—neuroprotective; anti-inflammatory, diuretic, and antibacterial.”Działanie lecznicze.

Turmeric preparations have been described as being used for liver failure, inflammation and poisoning, as well as viral hepatitis.

„Turmeric preparations are used to treat liver failure, gallstones, viral hepatitis, liver inflammation and poisoning, jaundice, and disorders of digestive juice secretion[3]. For medicinal use, turmeric rhizome should contain at least 2% curcuminoids, mainly curcumin, and at least 3–5% essential oil.”Preparaty kurkumowe są stosowane w leczeniu niewydolności wątroby, kamicy żółciowej, wirusowego zapalenia wątroby, stanów zapalnych i zatruć wątroby, żółtaczki i zaburzeń wydzielania soków trawiennych[3]. Kłącze ostryżu do celów leczniczych powinno zawierać nie mniej niż 2% kurkuminoidów, głównie kurkuminy i nie mniej niż 3-5% olejku eterycznego.

Henryk Różański recommends an herbal mixture to support the regeneration and detoxification of the liver and pancreas.

„Prescription. A protective mixture to support the regeneration and detoxification of the liver and pancreas. Ligularia herb or rhizome 25 g; dandelion herb 25 g; artichoke or cardoon herb 25 g; turmeric rhizome, powdered or finely chopped, 25 g; mint leaf 25 g; horehound herb or agrimony herb 25 g; elecampane root 25 g; everlasting herb or flower, or hawkweed herb or nipplewort herb 25 g. Mix the herbs.”Rp. Mieszanka ochronna i wspomagająca regenerację oraz odtruwanie wątroby i trzustki. Ziele lub kłącze języczka Ligularia 25 g Ziele mniszka lekarskiego 25 g Ziele karczocha lub ostrożenia warzywnego 25 g Kłącze w proszku lub krojone drobno kurkumy 25 g Liść mięty 25 g Ziele szanty lub ziele rzepika pospolitego 25 g Korzeń omanu 25 g Ziele lub kwiat kocanki, albo ziele jastrzębca lub pępawy 25 g Zioła wymieszać.
Hubert Czerniak

Hubert Czerniak lists silymarin from milk thistle among the remedies that may help cleanse the body.

„Other things that may help with cleansing include dandelion, cilantro (its leaves contain it), and Sylimarol (contained in a silymarin concentrate made from milk thistle seeds). The preparation Mumio, rich in vitamins and minerals, may also help cleanse the body. Children may also be given Lugol’s solution, which cleanses the body of heavy metals.”Pomocne w oczyszczaniu są także: mniszek lekarski, cilantro (zawiera ją nać kolendy), sylimarol (zawarty w koncentracie sylimarynowym z nasion ostropestu). W oczyszczaniu organizmu pomoże nam także preparat Mumio — bogactwo witamin i minerałów. Dzieciom warto podawać także płyn Lugola, który oczyszcza organizm z metali ciężkich.

Niruri leaf flower has been described as helpful for liver damage caused by alcohol, medicines, and chemical compounds, and for normalizing liver enzymes.

„Niruri leaf flower—has an effective action in liver damage caused by alcohol, medicines, and chemical compounds. It normalizes liver enzyme levels. Prepare an infusion from it (pour 200 ml of boiling water over 5 g of the herb and leave for 30 minutes. Drink twice a day).”Liściokwiat Niruri — wykazuje skuteczne działanie w uszkodzeniach wątroby przez: alkohol, leki, związki chemiczne. Normalizuje poziom enzymów wątrobowych. Przygotowujemy z niego napar (5 g ziela zalać 200 ml wrzątku, pozostawić na 30 minut. Pić 2 razy dziennie).
Henryk Różański

Henryk Różański points out that adding turmeric to food does not justify attributing liver-protective effects to it.

„It is unacceptable for the addition of turmeric to food to become an argument for claiming that such food will protect liver tissue and inhibit the growth of pathogenic bacteria and fungi. The same goes for wine: even a barely red, cheap plonk claims medicinal properties.”Niedopuszczalne jest, aby dodatek kurkumy (ostryżu) do żywności stał się argumentem pozwalającym na twierdzenie, iż taka żywność ochroni miąższ wątroby i zahamuje rozwój bakterii oraz grzybów patogennych. Tak samo jest z winem, byle sikacz ledwo czerwony przypisuje sobie właściwości lecznicze.

A meta-analysis of studies in people with nonalcoholic fatty liver disease found that silymarin lowered the enzymes ALT and AST, which are markers of liver damage.

Aim of the study: Nonalcoholic fatty liver disease (NAFLD) is a chronic liver disease with a high prevalence worldwide and poses serious harm to human health. There is growing evidence suggesting that the administration of specific supplements or nutrients may slow NAFLD progression. Silymarin is a hepatoprotective extract of milk thistle, but its efficacy in NAFLD remains unclear.

Results: A total of 26 randomized controlled trials involving 2,375 patients were included in this study. Administration of silymarin significantly reduced the levels of TC (SMD[95%CI]=-0.85[-1.23, -0.47]), TG (SMD[95%CI]=-0.62[-1.14, -0.10]), LDL-C (SMD[95%CI]=-0.81[-1.31, -0.31]), FI (SMD[95%CI]=-0.59[-0.91, -0.28]) and HOMA-IR (SMD[95%CI]=-0.37[-0.77, 0.04]), and increased the level of HDL-C (SMD[95%CI]=0.46[0.03, 0.89]). In addition, silymarin attenuated liver injury as indicated by the decreased levels of ALT (SMD[95%CI]=-12.39[-19.69, -5.08]) and AST (SMD[95% CI]=-10.97[-15.51, -6.43]). The levels of fatty liver index (SMD[95%CI]=-6.64[-10.59, -2.69]) and fatty liver score (SMD[95%CI]=-0.51[-0.69, -0.33]) were also decreased. Liver histology of the intervention group revealed significantly improved hepatic steatosis (OR[95%CI]=3.25[1.80, 5.87]). Silymarin can regulate energy metabolism, attenuate liver damage, and improve liver histology in NAFLD patients.

In the review, most studies reported lower liver enzyme levels after silymarin use, although the results varied.

Aim of the study: Silymarin, extracted from milk thistle (Silybum marianum), is esteemed for its antioxidative, anti-inflammatory, and antifibrotic properties, notably within liver-related contexts. Nevertheless, a comprehensive grasp of its effects on liver enzymes remains elusive.

Results: Nevertheless, outcome disparities may stem from comorbidities, suboptimal doses, and underlying diseases. Notably, silymarin's impact on liver enzymes could be context-dependent, with varying responses among different conditions, with the decrease of liver enzyme levels in patients with non-alcoholic fatty liver disease. Silymarin supplements exhibit potential for hepatoprotection by ameliorating liver enzyme levels across diverse conditions. Further research should ascertain optimal dosages and contexts, accounting for individual patient characteristics and underlying diseases.

In a small study of people with fatty liver, a mixture containing milk thistle improved the degree of fatty liver on ultrasound more often than a placebo.

Aim of the study: Metabolic dysfunction-associated steatotic liver disease (MASLD) lacks treatment options, with few evidence-based choices beyond lifestyle modification. This is the first clinical study to assess the efficacy of an organic liquid oral nutraceutical composed of turmeric extract, dandelion powder, milk thistle extract, and ginger powder in MASLD.

Results: Fifteen patients each were enrolled in the test product and placebo groups. The mean [standard deviation (SD)] age was 40.6 (10.8) years and 83.3% were male. A significantly higher proportion of patients in the nutraceutical product group had fatty liver grade improvement at Day 60 vs. placebo [60.0% (9/15) vs. 13.3% (2/15); p = 0.008]. Mean (SD) change in fatty liver grading was -0.60 (0.51) and -0.13 (0.35) for test product vs. placebo (p = 0.007). Adverse events were mild with similar incidence in both groups. The results are limited by the small sample size, single-center design, and short study duration.

In healthy people, a mixture of milk thistle and other herbs improved the results of several liver tests more than a placebo.

Aim of the study: Liver function is foundational to human health. While numerous herbal extracts have individually been shown to improve liver function, there are relatively few studies on combinations of herbal ingredients that may have synergistic effects. In addition, capsule fatigue is a common problem when aiming to incorporate multiple ingredients to support liver health.

Results: In total, 130 participants enrolled in the trial; 65 were randomized to each group. Mean improvements in ALT, AST, ALP, and GGT were significantly greater with the nutraceutical product than placebo (p ≤ 0.001). No adverse events were reported.

In a rat study, milk thistle combined with chicory and cumin, especially alongside HIIT training, improved indicators related to liver health and function.

Aim of the study: Fatty liver disease is a build-up of fats in the liver that can damage the organ and lead to serious complications. This study aimed to investigate the effects of exercise training and supplementation (milk thistle, chicory and cumin) on liver metabolites related to its function and health in rats with non-alcoholic fatty liver disease (NAFLD).

Results: The 4 NAFLD groups were subdivided and treated with (a) plain HFD, (b) high-intensity interval training (HIIT), (c) supplement (milk thistle, chicory, and cumin), and (d) combined HIIT and supplementation for 4 weeks. The induction of NAFLD through HFD yielded dyslipidemia, liver tissue damage, increased malondialdehyde, uncoupling protein 2 (UCP2), and phosphatidylinositol-3 kinase (PI3K), as well as decreased superoxide dismutase (SOD) and peroxisome proliferator-activated receptor gamma co-activator 1 alpha (PGC-1α) in liver tissue (p < 0.05). The 4 weeks intervention with either HIIT, supplement or especially the combined application of both, reversed these factors (p < 0.05) through changes in their concentrations in a direction indicative of enhanced liver health and function.

„The liver is the primary site of phase I and phase II biotransformation. There are many different routes of excretion, including urine, bile, sweat, saliva, tears, milk, and stool. By far, the most important excretory organs are the kidney and liver. The kidneys are an important organ involved in the elimination of drugs and their metabolites.
