Bleeding gums—a warning sign for the whole body?

Sources: 18 · Article: 23 975 characters

Igor Dogocki links chronic periodontitis to an increased risk of heart attack, stroke, and poorly controlled diabetes.

  • Bacteria from the mouth can enter the bloodstream and reach distant parts of the body.
  • Bleeding gums are associated with gum inflammation, while deficiencies in vitamins B2, B3, B6, and B12 are linked to hemorrhagic gingivitis and periodontitis.
  • Dogocki also lists hormonal disorders and heavy metal poisoning as possible associations with gum disease.

Igor Dogocki

Bleeding gums are associated with gum inflammation, which may occur alongside abnormal blood phosphorus levels.

„Gingivitis (which is associated with gum bleeding) usually occurs not only when proper oral hygiene is neglected, but also when gland function is disrupted, leading to increased phosphorus levels in the blood. The inflammation can be treated by balancing phosphorus levels with calcium levels.”Do zapalenia dziąseł (co wiąże się z ich krwawieniem) oprócz nieprzestrzegania należytej higieny, zazwyczaj dochodzi, gdy zaburzona zostaje praca gruczołów, co prowadzi do podwyższenia poziomu fosforu we krwi. Stan zapalny można wyleczyć poprzez zrównoważenie poziomu fosforu względem poziomu wapnia.

In periodontal disease, free radicals can damage tissues, and the blood can carry them to distant organs.

„An increase in free radicals causes oxidative damage to the gum tissue, periodontal ligament, and alveolar bone. They can also be carried by the blood to more distant structures and organs in the body. The damage caused by free radicals is regulated by the antioxidant defense system. When there is an imbalance between free radical production and antioxidant levels, this causes a condition known as oxidative stress (OS), defined as an imbalance between free radical production and the body's ability to eliminate these reactive species.”Wzrost ilości wolnych rodników powoduje uszkodzenia oksydacyjne tkanki dziąseł, ozębnej i kości wyrostka zębodołowego, a także z krwią są przenoszone na odleglejsze struktury i organy w organizmie.

Periodontal bacteria can enter the blood, reach distant parts of the body, and be associated with various systemic diseases.

„Periodontal pathogens can also enter the blood and circulate throughout the body, then colonize various distant anatomical sites. For example, periodontal bacteria are involved in several systemic diseases, including endocarditis and brain abscesses.”Patogeny przyzębia mają również zdolność przenikania do krwi i krążenia ogólnoustrojowego, a następnie kolonizacji różnych odległych miejsc anatomicznych w ciele. Na przykład bakterie przyzębia są zaangażowane w kilka chorób ogólnoustrojowych, w tym zapalenie wsierdzia i ropnie mózgu.
Igor Dogocki

Chronic periodontitis has been linked to an increased risk of atherosclerotic complications, heart attack, stroke, and poorly controlled diabetes.

„Chronic periodontitis in adults is associated with several conditions, including an increased risk of atherosclerotic complications, heart attack, stroke, poorly controlled diabetes, and most likely CA. The host response also plays an important role in causing systemic effects by producing many inflammatory mediators, including cytokines (in response to periodontal bacteria), which enter the systemic circulation.”Przewlekłe zapalenie przyzębia dorosłych wiąże się z kilkoma stanami, w tym zwiększonym ryzykiem powikłań miażdżycowych, zawałem mięśnia sercowego, udarem, źle kontrolowaną cukrzycą i najprawdopodobniej z CA.

Inflammatory molecules produced in the mouth can spread throughout the body, and Nathan S. Bryan links this process to an increased risk of heart attack and stroke.

„The cytokines and inflammatory molecules produced in the mouth in response to inflammation of the gums and mouth can travel throughout the body and wreak havoc on other systems. The net result is that all of the above disrupt nitric oxide production and, as a consequence, put your body at increased risk for heart attack and stroke.”The cytokines and inflammatory molecules produced in the mouth in response to the inflammation of the gums and mouth can travel throughout the body and wreak havoc on other systems. Igor Dogocki notes that healthy gums form a barrier that limits the uncontrolled passage of bacteria from the mouth into the blood.

„Healthy gums form a barrier that prevents bacteria in the mouth from entering the bloodstream uncontrollably. In this way, they provide protection similar to that provided by the skin. Gum problems are a disease of modern times. Our gums become diseased when we disrupt the body's chemical balance, for example.”Zdrowe dziąsła są barierą nie pozwalającą na niekontrolowane wnikanie bakterii znajdujących się w jamie ustnej do krwiobiegu. Tym sposobem pełnią rolę ochronną zbliżoną do tej, którą pełni skóra. Problemy z dziąsłami to choroba współczesności. Nasze dziąsła chorują, gdy zakłócamy równowagę chemiczną organizmu np.

Igor Dogocki lists bleeding at the front of the gums as a possible sign of heavy metal poisoning, and also links gum disease to hormonal disorders and leukemia.

„Other causes of gum disease may include bleeding in the front part of the mouth; heavy metal poisoning (e.g., mercury); hormonal imbalances (e.g., increased estrogen during pregnancy); significant gum overgrowth caused by cancers such as leukemia; plaque, which is a scourge for smokers; and heavy tartar buildup, which may occur in people who snore or sleep with their mouths open, or, of course, as a result of not cleaning the teeth and missing scheduled cleanings.”Do innych przyczyn chorób dziąseł możemy zaliczyć: krwawienie w odcinku przednim zatrucie metalami ciężkimi (np. rtęcią); rozregulowanie hormonalne (np. w ciąży wzrost estrogenu); znaczmy przerost dziąseł w wyniku chorób nowotworowych jak białaczka; osad jest zmorą palaczy a masywny kamień nazębny może występować u osób, które chrapią bądź śpią z otwartą buzią, czy oczywiście na skutek braku oczyszczania i nie przestrzegania terminów higienizacji.

Deficiencies in vitamins B2, B3, B6, and B12 are linked to hemorrhagic gingivitis and periodontitis.

„It has been proven that deficiencies in vitamins B2, B3, B6, and B12 are associated with hemorrhagic gingivitis and periodontitis. It has also been shown that vitamin B12 deficiency can lead to acute necrotizing gingivitis because vitamin B12 plays an important role in regulating cellular immunity.”Udowodniono, że braki witamin B2, B3, B6 i B12 są związane z krwotocznym zapaleniem dziąseł i zapaleniem przyzębia. Wykazano również, że niedobór witaminy B12 może prowadzić do ostrego, martwiczego zapalenia dziąseł, ponieważ odgrywa istotną rolę w modulacji odporności komórkowej.

Igor Dogocki reports that, in some patients diagnosed with gum disease, worsening bleeding was associated with impaired mitochondrial function in periodontal tissues.

„It has been shown that, in some patients with diagnosed gum disease, the progression of gum bleeding is caused by mitochondrial dysfunction in periodontal tissue cells. For this reason, one interesting treatment for gum disease is the use of CoQ10. Ubiquinol is the active, or “converted,” fat-soluble form of coenzyme Q10, which helps convert food into energy. In addition, CoQ10 is a powerful antioxidant that protects against damage caused by toxic free radicals.”Dowiedziono, że progresja krwawienia dziąseł w stwierdzonej chorobie dziąseł u niektórych pacjentów jest spowodowana dysfunkcją mitochondriów w komórkach tkanek przyzębia.

Igor Dogocki notes that harmful bacteria from the mouth can enter the blood and cause inflammation in the body.

„Exposure to dental metals: mercury from amalgam fillings Periodontitis and gingivitis—an infection caused by specific bacteria Herpes simplex—cold sores located on the lips, in the mouth, or around the face Root canal-treated teeth filled with materials containing a heavy metal—cadmium Harmful bacteria in the oral microbiome entering the bloodstream and causing inflammation in the body To prove the theory that neuroinflammation causes Alzheimer's disease, scientists isolated tissue from human brains and tested it for specific bacterial strains.”Ekspozycja na metale stomatologiczne : rtęć z wypełnień amalgamatowych Zapalenie przyzębia i dziąseł – infekcja wywołana przez specyficzne bakterie Herpes simplex – opryszczka umiejscowiona na ustach, w jamie ustnej lub okolicach twarzy Leczone kanałowo zęby wypełnione materiałami zawierającymi metal ciężki – kadm Zjadliwe bakterie w mikrobiomie jamy ustnej, przedostające się do krwioobiegu i wywołujące stany zapalne w organizmie W celu udowodnienia tezy neurozapalnej przyczyny choroby Alzheimera naukowcy wyizolowali tkanki z ludzkich mózgów i przetestowali na obecność określonych szczepów bakterii.

This case shows that persistent gum bleeding can point to the diagnosis of an inherited blood disorder.

Aim of the study: Von Willebrand disease (VWD) is not a rare inherited bleeding disorder. However, it is frequently underdiagnosed because early mucocutaneous bleeding manifestations are often attributed to local factors such as inflammation or anatomical abnormalities, leading to delayed recognition of the underlying systemic disease.

Results: This case highlights two important clinical messages. First, clinical practitioners, including dentists, can serve as key first responders in identifying systemic disorders by carefully evaluating bleeding and family histories rather than focusing solely on local oral conditions. Second, a comprehensive, patient-centered diagnostic approach is essential to avoid delayed diagnosis of inherited bleeding disorders such as VWD. This case report emphasizes the important role of non-hematology clinicians in the early diagnosis of rare VWD subtypes.

A review indicates that gum bleeding can be a symptom of systemic diseases, including blood disorders.

Aim of the study: On examination, the oral cavity may exhibit manifestations of underlying systemic disease and serve as an indicator of overall health. Systemic diseases with oral findings include autoimmune, hematologic, endocrine, and neoplastic processes.

Results: Patients with chronic diseases such as gastroesophageal reflux and eating disorders may present with dental erosions that cause oral pain or halitosis. In the pediatric population, oral changes can be related to rare cancers, such as Langerhans cell histiocytosis, or infectious etiologies, such as Kawasaki disease. In both adults and pediatric patients, poor oral health has been linked to poorer health outcomes overall. Thorough history taking and physical examination by dentists may aid in determining the underlying etiology of oral changes and allow for earlier intervention by medical colleagues.

The study found that persistent bleeding associated with periodontal disease was more common in older people and was linked, among other things, to hypertension and anticoagulant medications.

Aim of the study: Objective More patients visit oral surgery outpatient clinics from evening to night to treat persistent gingival bleeding (PGB) related to periodontal disease (PD) (PD-PGB). Since there are few reports on PD-PGB, the present study performed a detailed characterisation of this disease.

Results: Patients in the PGB-RPD group showed a significantly higher percentage (57.1%) of taking anticoagulants and/or antiplatelets than the control group (24.2%) (P < 0.001). A significantly higher percentage of patients in the PGB-RPD group had hypertension (68.1%) than in the control group (34.1%) (P < 0.001). Bleeding is most frequently observed in molars, with pocket depths exceeding 4 mm. Conclusion: PD-PGB accompanied by deep pockets was more common in older patients and aggravated by taking anticoagulants and/or antiplatelets and antihypertensives. PD-PGB is not rare, and periodontal maintenance, such as regular dental checks, is an important preventive measure.

In older patients with bleeding related to periodontal disease, elevated systolic blood pressure was independently associated with the bleeding.

Aim of the study: This study aimed to verify whether blood pressure was higher in patients presenting to the dental out-of-hours clinic with post-extraction bleeding (PEB) and periodontal disease-related gingival bleeding (PDB) compared to those presenting for other conditions (Control (C) group). Additionally, we examined differences in the prevalence of hypertension, antithrombotic use, and diabetes mellitus among the PEB, PDB, and C groups.

Results: A total of 416 bleeding patients were included, comprising 216 patients with PEB, 119 with PDB, and 81 with other bleeding. Because of the limited number of cases in the Y and M-groups, multivariable logistic regression analyses were performed only in the O-group (≥ 60 years). According to that, antithrombotic use was independently associated with both PDB and PEB in males and females. Increased SBP was independently associated with PDB in both sexes and with PEB in males. Hypertension remained independently associated with PDB only in females, whereas diabetes mellitus was not independently associated with either bleeding condition. Particularly among the O-group (≥ 60 years), antithrombotic use was independently associated with both PDB and PEB, whereas increased systolic blood pressure was independently associated with PDB and, to a lesser extent, PEB.

A meta-analysis found that obstructive sleep apnea is associated with more frequent periodontitis and greater bleeding during gum examination.

Aim of the study: The present meta-analysis aimed to evaluate quantitively the recent scientific evidence regarding the association between obstructive sleep apnea (OSA) and periodontitis.

Results: Thirteen eligible studies comprising a total of 31,800 patients were included. The meta-analysis showed an increased prevalence of periodontitis in OSA populations compared to controls. Both PD and CAL were increased in OSA populations compared with controls. (Prevalence of periodontitis: OR 2.348; 95%CI 2.221-2.482; PD: SMD = 0.681, 95% CI: 0.062-1.301, Z = 2.61, P = 0.031; CAL: SMD = 0.694, 95% CI: 0.167-1.22, Z = 2.58, P = 0.01). The study also found significantly increased BOP in patients with OSA after heterogeneity was clarified. (SMD = 0.357, 95% CI: 0.079-0.635, Z = 2.52, P = 0.012). The findings suggest that OSA was associated with an increased prevalence of periodontitis.

A meta-analysis found that Crohn's disease is associated with more frequent and severe periodontitis, including greater bleeding during examination.

Aim of the study: Crohn's disease (CD) is a chronic inflammatory disorder with systemic manifestations, including potential effects on oral health. Evidence regarding its association with periodontal disease remains inconsistent.

Results: Thirty-four studies, including 6482 CD patients and 9137 controls, were analyzed. CD was -associated with a higher risk of periodontitis (OR = 2.14, 95% confidence interval [CI]: 1.65-2.77). Clinical periodontal parameters were also significantly worse in CD patients: probing depth (SMD = 0.42), clinical attachment loss (CAL) (SMD = 0.50), bleeding on probing (SMD = 0.47), plaque index (SMD = 0.39), and gingival index (SMD = 0.31). Associations were stronger in European populations and in studies using CAL criteria. Sensitivity analyses confirmed the robustness of results. CD is associated with increased prevalence and severity of periodontal disease. These -findings support routine periodontal screening and integrated multidisciplinary management for CD patients, highlighting shared inflammatory pathways as potential therapeutic targets.

A review indicates that gum disease is associated with an increased risk of cardiovascular disease and type 2 diabetes.

Aim of the study: Examination of the oral cavity can identify clinical signs indicative of underlying systemic disease. Key features to examine include the general appearance and number of the teeth, signs of inflammation of the mucosa or gingival tissues including bleeding of the gums and redness, swelling or hyperplasia.

Results: The aim of this narrative review is to outline both the impact of periodontitis and xerostomia on cardiometabolic disease and the impact of cardiometabolic health on these oral health conditions. Key features of periodontitis and xerostomia will be provided along with a brief discussion of current concepts in early prevention and management of these oral health conditions. The biological mechanisms linking cardiometabolic disease and periodontitis will be outlined and the evidence supporting the association between cardiometabolic disease and oral health conditions will be presented together with an identification of areas where further research is indicated. Last, guidance for general practitioners to assess and support early diagnosis and management of oral health conditions by raising awareness of the relationship between oral health and cardiometabolic disease, providing simple oral health advice and referring to a dental practitioner will be presented.

A review lists diabetes and certain other systemic diseases as risk factors for moderate or severe periodontitis.

Aim of the study: Assessment of risk for periodontitis is still in its infancy. Nevertheless, a sufficient amount of dependable information exists to begin using risk assessment in the day to day practice of dentistry.

Results: Some risks such as pathogenic bacteria in the subgingival flora are strongly linked to causation of the disease while others such as bleeding on probing may indicate enhanced risk for future disease but are not known to be involved in causation and still others such as advancing age may be background factors that enhance susceptibility. While some risks such as cigarette smoking can be modified to lower the level of risk, others such as ageing are immutable and cannot be modified but need to be considered in overall risk assessment. A goal of periodontal diagnosis, treatment planning and therapy is to lower risk for future periodontal deterioration to the maximal extent. One approach to achieving this goal is described.

In a 17-year-old patient, bleeding and gum enlargement occurred alongside PCOS, and dental and gynecological treatment reduced the acute inflammation.

Aim of the study: Polycystic ovary syndrome (PCOS) is a common endocrinopathy affecting reproductive-aged women, characterised by metabolic abnormalities, polycystic ovaries, hyperandrogenism, and chronic anovulation. This pathology has been attributed to hormonal imbalances, including elevated levels of insulin, growth hormone (GH), ghrelin, LEAP-2, GnRH, LH/FSH ratio (>2:1), androgenic hormones, and estrogen.

Results: This case highlights that PCOS may contribute to or exacerbate gingival enlargement through hormonal and inflammatory alterations rather than being a direct etiological factor. Early interdisciplinary diagnosis and co-management between dental and medical professionals are crucial for optimizing both periodontal and systemic health outcomes in adolescent patients.

In pregnant women, greater gum bleeding was associated with changes in salivary markers of inflammation and the body's response.

Aim of the study: Pregnancy increases susceptibility to gingival inflammation, and saliva may help characterize associated host-response profiles. This cross-sectional study compared nine salivary proteins and inflammatory mediators between pregnant women with high and low gingival bleeding burdens.

Results: Primary comparisons used ANCOVA adjusted for maternal age and total protein concentration, with additional adjustment for visible biofilm in sensitivity analyses. In the primary adjusted analyses, G1 showed higher RBP4 (FDR-adjusted p = 0.004; partial η2 = 0.466) and clusterin (FDR-adjusted p = 0.006; partial η2 = 0.413), and lower IL-10 (FDR-adjusted p = 0.032; partial η2 = 0.285). In the sensitivity analyses, significant group effects were observed for RBP4 (FDR-adjusted p = 0.026; partial η2 = 0.381) and IL-8 (FDR-adjusted p = 0.044; partial η2 = 0.303). These findings demonstrate associations between high gingival bleeding burden and a salivary profile involving inflammatory/chemotactic, regulatory, and stress-response signals.

„Ads for both drugs warn that they increase the risk of bleeding. Obviously there are times when slowing coagulation is not a good thing, for example (as mentioned above) opening a child-proof package with a sharp object, slipping on the ice, banging your head hard which may cause bleeding in the brain (cephalhematoma). Also, inhibiting the clumping effect of platelets with a daily aspirin can bring about a similar result, but in many people aspirin taken long term is damaging to the gut. Commonly, both drugs are taken at the same time, which may increase the risk of bleeding.”Ads for both drugs warn that they increase the risk of bleeding.
