Foxglove — a beautiful flower that saves the heart or stops it?

Sources: 7 · Article: 12 846 characters

Foxglove strengthens the heart’s contractions, but at the same time slows its rhythm and the conduction of electrical impulses.

  • This helps the heart pump blood more strongly, and foxglove may increase urine output and reduce swelling.
  • However, it can be harmful: it may cause nausea, vomiting, and loss of appetite, and an overdose may also cause vision problems and dizziness.
  • It is contraindicated in some heart conditions, and in cases of partial heart block it can lead to complete heart block.

Foxglove increases the tone and contractility of the heart muscle, and also slows the heart rate and conduction in the heart.

„Foxglove increases the resting tone of the heart muscle, slows the heart rate, slows conduction in the Palladino–His bundle, and increases the excitability and contractility of the heart muscle. It dilates the coronary arteries and narrows the hepatic and splenic arteries.”Naparstnica zwiększa napięcie mięśnia sercowego w spoczynku, zwalnia akcje serca, zwalnia przewodzenie w pęczku Palladino-Hissa, zwiększa pobudliwość oraz kurczliwość mięśnia sercowego. Rozszerza tętnice wieńcowe, zwęża tętnice wątrobowe i śledzionowe.

Foxglove glycosides strengthen the heart’s contractions, reduce the amount of blood left in the ventricles, and increase the amount of blood pumped out by the heart.

„They increase the tone and contractility of the heart muscle, as well as the strength, speed, and precision of its contractions (positive tonotropic and positive inotropic effects). They reduce the amount of blood left in the ventricles after contraction and increase the heart’s stroke volume.”Wzmagają napięcie i kurczliwość mięśnia sercowego, zwiększają siłę, szybkość i dokładność skurczu mięśnia sercowego (działanie tonotropowo dodatnie, inotropowo dodatnie). Zmniejszają ilość krwi zalegającej w komorach po skurczu. Zwiększają pojemność wyrzutową serca.

The mechanism of action described involves increasing the amount of calcium in heart cells, which strengthens the contraction of the muscle.

„The calcium concentration in the cytoplasm of muscle cells increases, activating the contractile systems. This results in the previously mentioned increase in the tone and force of contraction of the heart muscle. The role of calcium in this process was already known in the 1930s. Foxglove glycosides lengthen the relaxation pause, thereby slowing the heart.”W cytoplazmie komórek mięśniowych stężenie wapnia wzrasta, co aktywuje układy kurczliwe.

Foxglove dilates the coronary vessels, acts as a diuretic, and may reduce swelling associated with circulatory failure.

„It dilates the coronary arteries and narrows the hepatic and splenic arteries. It slightly lowers venous pressure and raises arterial pressure. It acts as a diuretic and reduces swelling caused by circulatory failure. The dose of powdered leaves is 0.3–0.6 g per day, until symptoms of heart disorders resolve; the maintenance dose is 0.2 g daily.”Rozszerza tętnice wieńcowe, zwęża tętnice wątrobowe i śledzionowe. Nieco zmniejsza ciśnienie żylne. Podnosi ciśnienie tętnicze. Działa moczopędnie. Zmniejsza obrzęki na tle niewydolności krążenia. Dawki sproszkowanych liści: 0,3-0,6 g na dobę, do ustąpienia objawów zaburzeń sercowych; dawka podtrzymująca 0,2 g dobowo.

Current evidence indicates that low doses of foxglove may reduce worsening of heart failure, but do not reduce mortality and require monitoring of drug levels.

Aim of the study: Cardiac glycosides have undergone one of the most paradoxical trajectories in cardiovascular therapeutics, evolving from foundational heart failure therapy to near abandonment in the modern era.

Results: Contemporary evidence now supports a more disciplined reassessment. Recent randomized trials, including Digitoxin to Improve Outcomes in Patients with Advanced Chronic Heart Failure (DIGIT-HF) and Digoxin Evaluation in Chronic Heart Failure: Investigational Study in Outpatients in the Netherlands (DECISION), together with a contemporary meta-analysis, demonstrate consistent directional reductions in worsening heart failure events with low-dose digitalis therapy, without excess mortality when serum concentrations are carefully controlled. These findings suggest that the historical toxicity narrative largely reflected excessive drug exposure rather than intrinsic biologic hazard. Digitalis glycosides should not be viewed as mortality-reducing "foundational" therapy but rather as complementary agents for selected patients with persistent residual risk despite optimized guideline-directed therapy, particularly those with recurrent decompensation or concomitant atrial fibrillation.

At low concentrations, foxglove may improve heart rate control and reduce congestion and flare-ups, while higher doses increase the risk of abnormal heart rhythms and death.

Aim of the study: Digitalis glycosides are among the oldest therapies for heart failure (HF) with reduced ejection fraction, yet their contemporary role remains debated.

Results: Finally, this review also provides valuable insights from a 2025 survey of Portuguese HF specialists into real-world clinical practice regarding digitalis use. With 55 experts responding, the survey highlights a predominant, selective application of digitalis glycosides, primarily after optimization of foundational HF therapies, consistent with guideline recommendations. Perceived benefits focus on rate control and symptom relief, while concerns about toxicity, the need for monitoring, and gaps in modern trial evidence continue to restrict broader adoption. Together, current data support a repositioning of digitalis as a low-dose, concentration-guided, neurohormonal modulator and rate-control agent for carefully selected patients with HF with reduced ejection fraction, an approach that ongoing and future trials should further refine.

Foxglove increases the force of the heart’s contractions, but age, illness, and electrolyte disturbances may narrow the safety margin and increase sensitivity to its toxic effects.

Aim of the study: Cardiac Na+,K(+)-ATPase, the receptor molecule for digitalis glycosides, have isoforms with different intrinsic affinities for the glycosides. Expression of these isoforms are under developmental and hormonal regulation.

Results: Reserve capacity of the sodium pump is also reduced by pathological conditions or aging, resulting in reduced margin of safety for the glycoside. Events that follow sodium pump inhibition also affect sensitivity of the heart to digitalis toxicity. These are hypercalcemia and magnesium depletion. It is now feasible to predict digitalis sensitivity of the heart, not empirically but based on the understanding of the mechanisms responsible for the positive inotropic and toxic actions of the glycoside.

In the DIGIT-HF study, digitoxin was associated with fewer deaths or hospitalizations due to heart failure.

Aim of the study: Cardiac glycosides represent one of the oldest therapeutic pillars in medicine, although their role has progressively diminished in the modern era of neurohormonal therapy for heart failure (HF).

Results: Following publication of the Digitalis Investigation Group (DIG) trial in 1997, no randomized clinical trials evaluating this drug class were conducted for almost three decades, while observational studies and meta-analyses accumulated, often yielding conflicting results. The DIGIT-HF randomized clinical trial (2025) addressed a major unmet need by evaluating digitoxin in patients with HF with reduced ejection fraction in the contemporary era of guideline-directed medical therapy. Treatment with digitoxin was associated with favourable outcomes, including a reduction in the composite endpoint of all-cause mortality and hospitalizations for HF, thereby reopening a long-standing debate on the role of digitalis, which had remained marginal for many years. This article reviews the history of digitalis, the main evidence generated over recent decades, the evolution of clinical guidelines from 1997 to 2022, and the conceptual and clinical implications of the DIGIT-HF trial.
