Sources: 12 · Article: 18 023 characters

The European Medicines Agency considers that willowherb herb, consisting of the aerial parts of Epilobium parviflorum, may relieve urinary problems associated with benign prostate enlargement.

  • Its use is based on tradition, and laboratory studies do not provide conclusive evidence.
  • A doctor should rule out serious conditions before use; no adverse effects were reported in the HMPC assessment.
  • Henryk Różański recommends willowherb or fireweed as an ingredient in herbal mixtures for prostate enlargement and urinary problems.

What the authors write

Europejska Agencja Leków (EMA)

Willowherb herb consists of the aerial parts of Epilobium parviflorum.

„Willowherb herb is the common name for the aerial parts of Epilobium angustifolium L. and/or Epilobium parviflorum Schreb., collected before or during flowering. Willowherb herbal preparations are made by drying and chopping (breaking into small pieces) the aerial parts of the plant.”Ziele wierzbownicy to nazwa zwyczajowa dla nadziemnych części rośliny Epilobium angustifolium L. i/lub Epilobium parviflorum Schreb., zebranych przed okresem kwitnienia lub w jego trakcie. Przetwory z ziela wierzbownicy uzyskuje się przez suszenie i rozdrabnianie (kruszenie na drobne cząstki nadziemnych części rośliny.
Europejska Agencja Leków (EMA)

Willowherb herb may relieve urinary problems associated with benign prostate enlargement.

„The HMPC concluded that, based on its long-standing use, willowherb herb may be used by patients with benign prostatic hyperplasia (BPH) to relieve lower urinary tract symptoms, such as difficulty starting urination or the need to urinate frequently.”HMPC ustalił, że z uwagi na wieloletnie zastosowanie ziele wierzbownicy może być stosowane przez pacjentów z łagodnym rozrostem gruczołu krokowego (ang. benign prostatic hyperplasia, BPH) do łagodzenia objawów ze strony dolnego odcinka układu moczowego takich jak trudność w rozpoczęciu oddawania moczu bądź potrzeba częstego oddawania moczu.
Europejska Agencja Leków (EMA)

A doctor should rule out serious conditions before using willowherb herb.

„Willowherb herb should be used only after a doctor has ruled out serious conditions. The use of these medicines is based on tradition because there is not enough evidence from clinical studies.

„The HMPC's conclusions on using medicines containing willowherb herb to relieve lower urinary tract symptoms associated with BPH are based on their “traditional use” for this purpose. This means that, although evidence from clinical studies is insufficient, these herbal medicines are considered effective and there is evidence that they have been used safely in this way for at least 30 years (including at least 15 years in the EU).”Ustalenia HMPC dotyczące stosowania leków zawierających ziele wierzbownicy do łagodzenia objawów ze strony dolnego odcinka układu moczowego związanych z BPH opierają się na ich „tradycyjnym stosowaniu” w takim wskazaniu.

Laboratory studies suggest a possible effect on prostate cell growth, but their results are inconclusive.

„Laboratory study results suggest that willowherb herb may affect prostate cell growth and may have anti-inflammatory and pain-relieving effects. However, no firm conclusions can be drawn from these results. Therefore, the HMPC's conclusions on the use of medicines containing willowherb herb are based on their long-standing use.”Wyniki badań laboratoryjnych wskazują, że ziele wierzbownicy może wpływać na wzrost komórek stercza oraz może wykazywać działanie przeciwzapalne i łagodzące ból.

The HMPC assessment reported no adverse effects associated with the use of these medicines.

„No adverse effects associated with the use of these medicines had been reported by the time the HMPC carried out its assessment. More information about the risks associated with medicines containing willowherb herb, including precautions for their safe use, can be found in the monograph under the “All documents” tab on the Agency's website: ema.europa.eu/Find medicine/Herbal medicines for human use.”Do chwili przeprowadzenia przez HMPC oceny nie zgłoszono żadnych działań niepożądanych związanych ze stosowaniem tych leków.

In a laboratory study, willowherb extracts inhibited prostate cancer cell growth and reduced PSA secretion.

Aim of the study: Extracts from Epilobium sp. herbs have been traditionally used in the treatment of prostate-associated ailments.

Results: Additionally, ellagitannins from E. hirustum extract were shown to be transformed by human gut microbiota into urolithins. Urolithin C showed the strongest activity in inhibiting cell proliferation (IC50 = 35.2 ± 3.7 μM), PSA secretion (reducing PSA secretion to 100.7 ± 31.0 ng/ml) and arginase activity (reducing it to 27.9 ± 3.3 mUnits of urea/mg of protein). The results explain the activity of Epilobium extracts and support the use of Epilobium preparations to treat prostate diseases.

In a laboratory study, willowherb extracts increased the death of prostate cancer cells.

Aim of the study: The aim of this work was to determine the effect of standardized aqueous extracts from Epilobium angustifolium L., E. parviflorum Schreb. and E. hirsutum L. herbs on the apoptosis of hormone-dependent prostate cancer cells (LNCaP).

Results: Using the HPLC-DAD-MS/MS method, 38 constituents were characterized. The extracts contained significant amounts of oenothein B, as well as flavonoids and phenolic acids. Exposure of LNCaP cells to the extracts (20, 50 and 70 μg/ml) resulted in a significant increase in the proportion of apoptotic cells, from 2.86 ± 0.5% in untreated cells to 86.6 ± 1.0%. All extracts significantly decreased mitochondrial potential, Δψm, resulting in an increase in caspase-3 activity from 0.3 ± 0.07 ng/mg of protein in untreated cells to 1.26 ± 0.32 ng/mg of protein. This study showed that Epilobium extracts are active against LNCaP prostate cancer cells and that their apoptotic activity is related to activation of the mitochondrial pathway. The high oenothein B content may influence the biological activity of these plant materials.

In laboratory studies, willowherb extract showed antioxidant activity, inhibited COX-1 and COX-2, and limited the growth of E. coli bacteria.

Aim of the study: Crude water and ethanolic extracts of five herbal remedies reported in the literature for traditional treatment of benign prostatic hyperplasia (BPH) and/or prostatitis were investigated for their effect on hydroxyl scavenging activity, antibacterial activity and their ability to inhibit cyclooxygenase-1 and -2 (COX-1 and COX-2) catalysed prostaglandin biosynthesis.

Results: All 10 extracts scavenged hydroxyl radicals, but with varying potency (32–93%). Ethanolic extracts were the most active in inhibiting COX-1-catalysed prostaglandin biosynthesis. The ethanolic extract of Epilobium parviflorum showed inhibitory effects on both COX-1- and COX-2-catalysed prostaglandin biosynthesis, inhibited the growth of Escherichia coli and had antioxidant activity. Although these results support the traditional use of Epilobium parviflorum to treat prostatitis and BPH, further investigation of this promising plant is needed.

In laboratory studies, willowherb extract showed antioxidant and anti-inflammatory effects.

Aim of the study: Epilobium parviflorum Schreb. (Onagraceae) is used for the treatment of benign prostatic hyperplasia (BPH), but its biological action is not entirely identified.

Results: E. parviflorum showed a greater antioxidant effect in the DPPH assay than well-known antioxidants and inhibited lipid peroxidation in the TBA assay (IC(50) = 2.37 +/- 0.12 mg/mL). At concentrations of 0.2-15.0 microg/mL, the extract protected fibroblast cells against oxidative damage to an extent comparable to catalase (250 IU/mL). In the COX inhibition assay, E. parviflorum decreased PGE(2) release, showing inhibition of the COX enzyme (IC(50) = 1.4 +/- 0.1 microg/mL).
