Amalgam — mercury in the mouth that must be removed safely?

Sources: 16 · Article: 19 052 characters

Igor Dogocki warns that unsafe amalgam removal may increase the amount of mercury entering the body.

  • Amalgam contains about half mercury, and the greatest amount is said to be released when a filling is placed or removed.
  • During removal, the patient and staff must be protected from inhaling vapors, using measures including a rubber dam, suction, masks, and a low-speed drill.
  • Piotr Witczak also describes ventilation, filters, protective clothing, and lowering the temperature during the procedure.

Igor Dogocki

Amalgam contains mercury, which makes up about half of its composition.

„Amalgam is a mixture of silver, tin, copper, and zinc, bonded together with mercury. Mercury makes up the largest part of it (about 50%). According to the World Health Organization (WHO), there is no safe limit for mercury; even the smallest amount is harmful.”Amalgamat jest mieszanką srebra, cyny, miedzi i cynku, które są połączone rtęcią. Główną jego część stanowi rtęć (około 50%). Według Światowej Organizacji Zdrowia (WHO) nie ma bezpiecznej granicy dla rtęci , jest ona nawet w najmniejszej ilości szkodliwa.
Igor Dogocki

The greatest amount of mercury is said to be released when amalgam is placed or removed.

„Mercury vapor is released from amalgam fillings during chewing, teeth grinding, and drinking hot beverages. In addition, studies have confirmed that amalgam fillings directly affect the development of chronic gum disease (periodontitis). However, the greatest amount of mercury is released from amalgam when it is placed or removed, which is why safe amalgam removal, as practiced at Bio Clinic, is so important.”Uwalnianie oparów rtęci z amalgamatów odbywa się podczas żucia, zgrzytania zębami oraz picia gorących napojów.

Igor Dogocki warns that unsafe removal may increase the amount of mercury entering the body.

„Warm and acidic foods increase the cascade Some symptoms of mercury poisoning from amalgam: painful periods, menstrual irregularities Apathy, autism, and depression Fatigue, irritability Diarrhea, digestive problems rheumatic diseases, inflammation and joint pain, muscle weakness and body aches excessive hair growth feeling cold, cold hands and feet Because mercury is extremely toxic and is released in greater amounts when amalgam fillings are removed, there is a real risk that even more mercury will enter the body and worsen poisoning.”Ciepłe i kwaśne posiłki zwiększają kaskadę Niektóre objawy zatrucia rtęcią z amalgamatów: bolesne menstruacje, zaburzenia miesiączkowania Apatia, autyzm i depresja Zmęczenie, drażliwość Biegunka, problemy z trawieniem choroby reumatyczne, zapalenie i bóle stawów, osłabienie mięśni i bóle ciała nadmierne owłosienie uczucie zimna, zimne dłonie i stopy W związku z ogromną toksycznością rtęci i jej zwiększonym uwalnianiem podczas usuwania wypełnień z amalgamatu, powstaje realne zagrożenie, iż przeniknie ona w jeszcze większym stopniu do organizmu i spowoduje nasilenie zatrucia.
Igor Dogocki

During removal, the patient and staff must be protected from inhaling mercury vapor.

„The most important issue for both the patient and the dentist and their assistant is providing proper protection from inhaling mercury vapor released during drilling. Before safe amalgam removal begins, the patient takes preparations recommended by a Bio Clinic doctor.”Najważniejszą kwestią zarówno dla pacjenta, jak i stomatologa i jego asysty, jest zapewnienie właściwej ochrony przed wdychaniem ulatniających się podczas borowania oparów rtęci. Przed przystąpieniem do bezpiecznego usuwania amalgamatów pacjent przyjmuje zalecone przez lekarza Bio Clinic preparaty.
Igor Dogocki

The safety measures described include covering the face and airways, a rubber dam, suction, masks, and a low-speed drill.

„The patient's face and airways are covered. A rubber dam is used to isolate the tooth and cover the rest of the mouth so that amalgam particles do not enter the throat. Special suction and masks that absorb mercury are used. Amalgam is removed at a very low speed using special burs.”Twarz i drogi oddechowe pacjenta są okrywane.
Piotr Witczak

Piotr Witczak describes additional precautions, including ventilation, filters, protective clothing, and lowering the temperature during the procedure.

„The procedure includes, among other things, adequate ventilation and filters, protective clothing for staff and the patient, sealed respirator masks with suitable mercury filters for the dentist and assistant, connection to an external oxygen source through a nasal mask or cannula, a rubber dam, powerful suction, lowering the temperature during the procedure, and cutting the amalgam with a suitable bur.”Procedura obejmuje m.in. odpowiednią wentylację i filtry, odzież ochronną dla personelu i pacjenta, szczelne maski oddechowe z odpowiednimi filtrami rtęci dla lekarza i asysty, podłączenie pod zewnętrzne źródło tlenu przez maskę nosową lub wąsy, koferdam, silny ssak, obniżanie temperatury zabiegu, przecinanie amalgamatów odpowiednim wiertłem.
Igor Dogocki

After the procedure, Igor Dogocki describes rinsing the mouth, taking preparations that bind mercury, and later rebuilding the tooth.

„After the procedure, the tooth is disinfected and protected, and the patient rinses their mouth with a specially prepared rinse and takes preparations that bind mercury. After a period of recovery, the tooth is rebuilt using materials free of BPA and allergenic resins, or all-ceramic inlays.”Po zabiegu ząb zostaje odkażony i zabezpieczony, a pacjent specjalnie przygotowaną płukanką płucze jamę ustną oraz przyjmuje preparaty wiążące rtęć. Następnie ząb po okresie regeneracji jest odbudowany materiałami wolnymi od BPA i żywic alergizujących lub pełnoceramicznymi wkładami.
Igor Dogocki

Igor Dogocki writes that people with amalgam fillings are exposed to daily mercury release, although not everyone experiences symptoms.

„In summary, not everyone experiences symptoms of mercury poisoning, but anyone with amalgam fillings has mercury in their body and is exposed to its daily release.”Podsumowując nie wszystkie osoby odczuwają objawy zatrucia rtęci, natomiast każdy kto ma wypełnienia amalgamatowe, ma rtęć w organizmie i jest narażony na codzienne uwalnianie się jej.

Igor Dogocki notes that it should not be assumed that every metal in the mouth needs to be removed.

„Some people may experience local reactions, sensitivity, an unpleasant metallic taste, irritation of the oral lining, or a confirmed allergy to specific alloy components. That is why at Bio Clinic® we do not start from the assumption that “all metal must be removed.” We start by asking: Are the materials in your mouth suitable for you, and is there a reason to replace them?”U części osób mogą jednak występować miejscowe reakcje, nadwrażliwość, nieprzyjemny metaliczny smak, podrażnienia błony śluzowej lub potwierdzona alergia na określone składniki stopów.
Igor Dogocki

Symptoms or suspected reactions to metals need to be assessed, rather than automatically attributed to metals.

„Patients who come to us often mention: a metallic taste in the mouth, recurring irritation or redness of the oral lining, a burning sensation or discomfort in the mouth, skin reactions around the mouth, a confirmed allergy to nickel or other metals, the presence of many different metal alloys in the mouth, old amalgam fillings, a wish to switch to metal-free dentistry, a need to replace metal crowns, bridges, or implants with ceramic materials. These symptoms can have various causes, so they need to be properly assessed rather than automatically attributed to the presence of metal.”Pacjenci zgłaszający się do nas często zwracają uwagę na: metaliczny smak w ustach, nawracające podrażnienia lub zaczerwienienia błony śluzowej, uczucie pieczenia lub dyskomfortu w jamie ustnej, reakcje skórne w okolicy ust, potwierdzoną alergię na nikiel lub inne metale, obecność wielu różnych stopów metali w jamie ustnej, stare wypełnienia amalgamatowe, chęć przejścia na stomatologię bezmetalową, potrzebę zastąpienia metalowych koron, mostów lub implantów materiałami ceramicznymi. Takie objawy mogą mieć różne przyczyny, dlatego wymagają właściwej oceny, a nie automatycznego przypisywania ich obecności metalu.
Piotr Witczak

Piotr Witczak recommends safely removing amalgam fillings to end ongoing exposure to toxins in the context of MS/ALS.

„Given the scientific reports cited above, a rational approach should primarily involve ending ongoing exposure and avoiding sources of toxins, especially heavy metals. To do this, amalgam fillings should be removed, but only safely and in accordance with appropriate procedures.”Z uwagi na przytoczone powyżej doniesienia naukowe racjonalne podejście powinno polegać przede wszystkim na przerwaniu bieżącej ekspozycji i unikaniu źródeł toksyn, w szczególności metali ciężkich. W tym celu należy usunąć plomby amalgamatowe, ale koniecznie w sposób bezpieczny, z zachowaniem odpowiednich procedur.
Igor Dogocki

Igor Dogocki describes individual recommendations for mercury removal therapy after the procedure.

„After the Safe Amalgam Removal procedure, the patient receives additional individual recommendations for therapy to remove mercury that has accumulated in the body over the years. The recovery phase begins once the amalgam has been safely removed.”Po przeprowadzonym zabiegu Bezpiecznego Usuwania Amalgamatu pacjent otrzymuje dodatkowe indywidualne zalecenia dotyczące terapii usunięcia rtęci, skumulowanej przez lata w organizmie. Od momentu Bezpiecznego Usunięcia Amalgamatu rozpoczyna się etap zdrowienia.
Igor Dogocki

Igor Dogocki presents a protocol that involves removing metals from the mouth, while emphasizing that safety measures must be followed.

„Metals and heavy metals alone can cause chronic illness that may lead to all of the following symptoms: Impaired cognitive function Anxiety and/or depression A metallic taste in the mouth Autoimmune diseases Peripheral neuropathy (numbness and tingling in the feet and hands) For the reasons listed above, the treatment protocol at BIO CLINIC Swiss Concept involves removing all metals from the mouth and replacing them with biocompatible, non-toxic, individually selected restorations in the form of suitable fillings, onlays, ceramic implants, and crowns. These procedures must, of course, be carried out with appropriate safety measures and support for the body.”Same metale i metale ciężkie mogą wywołać przewlekłą chorobę, która może powodować wszystkie następujące objawy: Upośledzenie funkcji poznawczych Lęk i / lub depresja Metaliczny posmak w ustach Choroby autoimmunologiczne Neuropatia obwodowa (drętwienie i mrowienie stóp i dłoni) Z powodów wymienionych powyżej, protokół leczenia w BIO CLINIC Swiss Concept zakłada usunięcie wszystkich metali z jamy ustnej oraz zastąpienie ich bio kompatybilnymi nie toksycznymi indywidualnie dobranymi odbudowami w postacie odpowiednich wypełnień, nakładów oraz implantów i koron ceramicznych. Jest rzeczą oczywistą, że należy te zabiegi przeprowadzić zgodnie z odpowiednimi środkami bezpieczeństwa i przy wsparciu organizmu.
Piotr Witczak

Piotr Witczak lists amalgam as one of many possible sources of mercury.

„Examples of significant sources of mercury include amalgam fillings, some vaccines, broken mercury thermometers and CFL bulbs, as well as prenatal exposure (transfer of heavy metals from the mother's body to the fetus) and less significant sources such as fish and seafood (especially large predatory fish), tattoos, and many others.”Przykładowe, istotne źródła rtęci to wypełnienia amalgamatowe, niektóre szczepionki, rozbite termometry rtęciowe i żarówki typu CFL, a także ekspozycja prenatalna (transfer metali ciężkich z organizmu matki do płodu) oraz mniej znaczące źródła, takie jak ryby i owoce morza (w szczególności duże ryby drapieżne), tatuaże i wiele innych.

A review found that oral lichenoid lesions often resolved or improved after amalgam removal.

Aim of the study: This systematic review summarizes the characteristics and treatment outcomes of dental amalgam-associated oral lichenoid lesions (OLLs) and oral lichen planus (OLP). Embase and MEDLINE were searched for original studies on OLLs or OLP associated with dental amalgam.

Results: Data were extracted from 44 studies involving 1,855 patients. Removal of amalgam restorations led to complete resolution in 54.2% (n = 423/781), partial resolution in 34.8% (n = 272/781), and no resolution in 11.0% (n = 86/781) of patients with OLLs. Among patients with OLP, complete resolution occurred in 37.1% (n = 72/194), partial resolution in 26.3% (n = 51/194), and no resolution in 36.6% (n = 71/194). For patients with OLLs, 91.6% of those with positive patch tests and 82.9% of those with negative patch tests improved after amalgam removal. For patients with OLP, the corresponding figures were 89.2% and 78.9%. Our results suggest that improvement occurs regardless of patch test status.

After amalgam removal, levels of inorganic mercury and silver in serum decreased, as did the severity of reported complaints.

Aim of the study: A prospective cohort study on changes in health complaints after removal of amalgam restorations was carried out at the request of the Norwegian Directorate of Health. Its aim was to provide and evaluate experimental treatment for patients with health complaints attributed to dental amalgam fillings.

Results: Concentrations of I-Hg and Ag in serum decreased significantly after removal of all amalgam restorations. Concentrations of MeHg and Se in serum did not change. The severity of health complaints was significantly reduced after amalgam removal, but there were no statistically significant correlations between exposure indicators and health complaints. After all amalgam restorations were removed, concentrations of I-Hg and Ag in serum decreased. The results support the hypothesis that exposure to amalgam fillings increases the daily dose of both I-Hg and Ag. Although the severity of health complaints decreased after all amalgam restorations were removed, there was no clear evidence of a direct relationship between exposure and health complaints. Trial registration: The project is registered at ClinicalTrials.gov (NCT01682278).

After the fillings were removed, mercury concentrations in saliva fell noticeably, and levels in the samples tested continued to decline over time.

Aim of the study: The toxicological consequences of exposure to mercury (Hg) from dental amalgam fillings are a matter of debate in several countries. This study aimed to obtain data on Hg concentrations in saliva and feces before and after removal of dental amalgam fillings.

Results: In saliva, Hg concentrations declined exponentially during the first 2 weeks after amalgam removal (t 1/2 = 1.8 days). The study concluded that amalgam fillings are a significant source of Hg in saliva and feces. Hg levels in all media decreased considerably after amalgam removal. Uptake of amalgam mercury in the gastrointestinal tract in connection with removal of amalgam fillings appears to be low.

In patients with mercury hypersensitivity, amalgam removal was associated with a decrease in the levels of two antibodies linked to autoimmune thyroiditis.

Aim of the study: The impact of dental amalgam removal on the levels of anti-thyroid peroxidase (anti-TPO) and anti-thyroglobulin (anti-Tg) antibodies was studied in patients with autoimmune thyroiditis (AT), with and without mercury allergy.

Results: Compared with levels at the beginning of the study, only patients with mercury hypersensitivity who underwent amalgam replacement (Group IIA) showed a significant decrease in both anti-Tg (p=0.001) and anti-TPO (p=0.0007) autoantibodies. Autoantibody levels did not change in patients with or without mercury hypersensitivity (Groups I and IIB) who did not have their amalgam replaced. Removal of mercury-containing dental amalgam in patients with mercury hypersensitivity may contribute to successful treatment of autoimmune thyroiditis.

One year after amalgam removal, mercury concentrations in plasma and urine were lower than before the procedure.

Aim of the study: In 10 healthy people, all amalgam fillings were replaced with gold inlays. Blood and urine levels were measured on 10 occasions during a 4-month period before and a 12-month period after amalgam removal.

Results: Twelve months after removal, plasma and urinary mercury levels in the experimental group had fallen significantly to 50% and 25%, respectively, of their initial values. Apart from significantly lower plasma selenium values 5 and 10 days after removal, no significant differences were found in plasma selenium or erythrocyte glutathione peroxidase, either within or between the experimental and control groups. A large number of additional biochemical analyses showed no effects on organ function and no differences between the groups before or after amalgam removal. Amalgam fillings made a considerable contribution to plasma and urinary mercury levels.

During amalgam removal, using water and suction together was associated with lower mercury vapor concentrations in the air than using suction alone.

Aim of the study: Amalgam used for dental fillings contains approximately 50% elemental mercury. During dental student training, amalgam is often removed by drilling without water spray or suction, which are protective measures against mercury aerosol. In this study, mercury vapor levels in ambient air were measured during amalgam removal as typically performed in dental training.

Results: When water spray and suction were used, mercury vapor levels ranged from 4.0 to 19.0 μg/m3 (arithmetic mean = 8.0 μg/m3); when suction alone was used, levels ranged from 14.0 to 999.0 μg/m3 (999.0 μg/m3 represents the Jerome analyzer's upper detection limit) (arithmetic mean = 141.0 μg/m3); when neither suction nor water was used, levels ranged from 34.0 to 796.0 μg/m3 (arithmetic mean = 214.0 μg/m3). The Alberta Occupational Health and Safety threshold limit value for mercury vapor over an eight-hour time-weighted period is 25.0 μg/m3. The absolute ceiling for mercury vapor, which must not be exceeded at any time, is 125.0 μg/m3. When both water spray and suction were used, mercury vapor levels were consistently below this threshold. When suction without water spray was used, levels exceeded the safety threshold 8% of the time. When neither water spray nor suction was used, 36% of mercury vapor readings exceeded the absolute ceiling.
