Lipiodol, an iodinated poppy seed oil used in lymphangiography, may help seal lymphatic leaks.

Aim of the study: Lipiodol is an iodinated poppy seed oil first synthesized in 1901. Originally developed for therapeutic purposes, it has mainly become a diagnostic contrast medium since the 1920s.

Results: In these indications, Lipiodol alone has a therapeutic effect with clinical success in 51% to 100% of cases. The 2 main access sites to the lymphatic system for lymphangiography are cannulation of lymphatic vessels in the foot (transpedal) and direct puncture of (mainly inguinal) lymph nodes (transnodal). In case of failure of lymphangiography alone to occlude the leaking lymphatic vessel as well as in indications such as protein-losing enteropathy, postoperative hepatic lymphorrhea, or plastic bronchitis, lymphatic vessels can also be embolized directly by injecting a mixture of Lipiodol and surgical glues (most commonly in thoracic duct embolization). The aim of this article is to review the historical role of Lipiodol and the evolution of its clinical application in lymphangiography over time until the current state-of-the-art lymphatic imaging techniques and interventions.

In women with recurrent implantation failure, an infusion of poppy seed oil before embryo transfer was associated with a higher clinical pregnancy rate.

Aim of the study: Oil-based contrast medium in hysterosalpingography has been recommended as an approach to improve fertility in recent years. However, this approach has rarely been applied to patients with unknown recurrent implantation failure (RIF). This randomized clinical trial aimed to assess the effects of intrauterine oil-based contrast infusion on frozen embryo transfer (FET) prognosis and pregnancy outcomes among patients with unknown RIF.

Results: One hundred patients completed intrauterine oil-based medium infusion followed by FET within 120 days. The intention-to-treat analysis demonstrated balanced baseline characteristics between groups. The oil-based medium group exhibited significantly higher CPR (56% vs. 37.4%, rate ratio (RR) 1.498, 95% CI 1.109-2.023, P = 0.007) and implantation rates (median 50% [interquartile range, IQR 0-100%] vs. median 50% [IQR 0-50%], P = 0.010) compared to controls. However, no statistically significant differences were observed in LBR (39% vs. 27.1%, RR 1.439, 95% CI 0.969-2.138, P = 0.069) or OPR (40% vs. 27.1%, RR 1.476, 95% CI 0.997-2.168, P = 0.049), with the latter approaching borderline significance. Subgroup analyses revealed enhanced CPR specifically in RIF patients aged 35-39 years (P = 0.041) and with gonadotropin-releasing hormone agonist (GnRHa) pretreatment hormone replacement therapy (HRT) (GnRHa-HRT) endometrial preparation protocol (P = 0.008).

In a study of women who underwent HSG using iodinated poppy seed oil, more than half of the participants became pregnant.

Aim of the study: Ethiodized poppy seed oil for hysterosalpingography (HSG) is reported to display some therapeutic effect on infertility, but big a sample-size study under real clinical settings is still lacking to verify the speculation. Thus, this real-world study enrolled 1,053 infertile patients who underwent ethiodized poppy seed oil-based HSG to explore its fertility enhancement value.

Results: The 3-, 6-, 12-month and total pregnancy rate was 22, 36.8, 50, and 53.8%, respectively. The total live birth rate was 42.7%. Sub-group analyses showed that pregnancy rate was 53.7, 53.8, 54.1, and 62.4% in subgroups of primary infertility patients, secondary infertility patients, infertility patients with fallopian tube disease, and infertility patients with unknown cause, respectively. Meanwhile the live birth rate was 44.3, 41.3, 41.5, and 59.2% in these subgroups, separately. Multivariate logistic regression analysis disclosed that BMI ≥ 24 kg/m2, history of dysmenorrhea, and abnormity of sperm count or motility-related infertility were independently correlated with reduced pregnancy rate and livebirth rate (All Ps < 0.05). Adverse events mainly included pain (20.6%) and interstitial reflux (7.9%), which were mild and tolerable.

In mice, an oil-based contrast medium administered into the uterus improved fertilization and birth rates after insemination with a low number of sperm.

Results: First, females were inseminated with a high number of sperm (5 × 10e5 spermatozoa). Comparable fertilization rates (67.0 ± 5.4% and 60.0 ± 8.0%, P > 0.05) were observed from oocytes originating from OSCM and PBS-administered mice, respectively. This showed that when inseminating with a high number of sperm, uterine OSCM administration had no deleterious effect but did not improve fertilization. We postulated that the potential beneficial effect of OSCM administration might be masked by the optimal reproductive system of young mice. It was therefore decided to test the effect of OSCM in suboptimal reproductive conditions such as those observed when sperm concentration is low (oligozoospermia). As could be expected, fertilization rate decreased proportionally to the reduction of inseminated sperm suggesting that it is a relevant model for oligozoospermia and to mimic subfertility.

In rats, poppy seed oil improved antioxidant defense markers after cerebral ischemia and reperfusion, but did not prevent a decrease in the number of hippocampal cells.

Aim of the study: The brain is highly sensitive to hypoxia; this is true particularly of parts that are crucial for cognitive function. The effects of hypoxia are especially dramatic in the hippocampus.

Results: The serum nitric oxide levels in CIR groups were elevated significantly compared to controls, and were significantly higher in group 2 than in group 3. The glutathione levels were increased significantly in the poppy seed oil treated group compared to the saline CIR groups. The malondialdehyde levels were markedly increased in group 3 compared to both groups 1 and 2. Our study suggests that poppy seed oil can improve antioxidant defense capacity after CIR, although this treatment did not alter significantly the frequency of cell death.
